The oroantral fistula (OAF) is the persistent communication between maxillary sinus and the oral cavity, occurring most often after extraction molar teeth due to its proximity to the maxillary sinus. Headache, sinusitis, pain, swallowing disorders and nocturnal cough are some of the reported symptoms. Less than 2mm diameter OAF tend to close spontaneously in most cases without the need for surgical approach. 3mm OAF or greater cases require surgical intervention. Surgical techniques using flaps (palatal, buccal or combined), bone graft and pedicle graft of buccal fat pad have been employed for this purpose. This paper aims to describe a clinical case of OAF treated with the technique of pedicle graft of buccal fat pad, focusing on their anatomy and discussing the advantages and limitations regarding the most used techniques.
