Introduction: Tooth trauma consists in an injury of severity, extent and intensity varied due to forces acting on the tooth element with high prevalence in children aged 1-3 years. Objective: To report a clinical case of a patient 1 year and 8 months, male, attended in the pediatric dentistry clinic of the Federal University of Amazonas, complaining of fractured tooth 51. Method: After clinical and radiographic examination, was diagnosed the presence of a crown-root fracture with great extension. Result: The treatment of choice was the extraction, since the conservative treatment was impracticable due to subgingival extension of the lesion. The install of the appliance space maintainer was not indicated. Conclusion: The dental trauma, especially in infants, is a challenge for the dental surgeon. The approach of a root-crown fracture should be based on common sense and scientific knowledge to obtain appropriate treatment.
