The rapid expansion of surgically assisted maxillary (ERMCA) is accomplished through osteotomies of the pillars of the maxilla and palatal suture, promoting the correction of transverse maxillary deficiency. Less invasive techniques have been described, aiming at performing these procedures under local anesthesia, eliminating the costs of hospital and bringing comfort to patients. The purpose of this paper was to discuss the issues involved in facilitating ERMCA technique under local anesthesia through a case report of a 27 years old patient. It is concluded that the diagnosis by clinical evaluation and study models is essential for the indication of ERMCA and this procedure provides good predictability in the correction of transverse deficiency, with minimal morbidity. Furthermore, the use of a minimally invasive technique is effective in the treatment of maxillary atresia, with recurrence rates similar to those more invasive techniques.
