The endodontic treatment of a nonvital tooth with an incomplete root formation shows difficulties for the instrumentation and for the good obturation. The main purpose of this article is to present a case report in which, through the apexification technique, the treatment of an immature nonvital tooth is successful. Besides, it shows a bibliographic research about the subject and discusses some relevant points. It is also pointed out the importance of an esthetic rehabilitation and the reinforcement of a dental structure, once a tooth with its incomplete root formation becomes very sensitive to the fracture. The case is about an immature maxillary central incisor, in which a traumatic incident caused a fracture and, consequently, pulp necrosis. Through exchanges of calcium hydroxide paste, it was possible to induce the apical closure, allowing the conclusion of the endodontic treatment. Next, the restoration with the use of a reminiscent fragment of the tooth and the remaining reconstruction with light cure compost resin were accomplished. It is concluded that the apexification is a consecrated technique to treat open apices, with easy achievement, well accepted by the patients and showing a great success level.
