OBJECTIVE: The aim of this study was to evaluate the post-operative pain in vital teeth with one or two roots, whether or not submitted to the enlargement of the apical foramen up to file 30. METHODS: Forty teeth were selected and randomly divided into two groups. In Group I (experimental) the foramen enlargement up to file 30 was recommended and the canal instrumented from 1 to 2 mm short beyond the apical foramen. In Group II (control) the canal was prepared from 1 to 2 mm short beyond the apical foramen and the pulpar tissue was preserved. All procedures were accomplished by the same operator. The apex locator Root ZX® was used associated with radiographies in order to get the adequate root canal length. All the teeth received the root canal treatment in one appointment and the pulp vitality based on clinical and radiographic aspects. The pain was evaluated over a period of 24 and 48h after root canal filling. RESULTS AND CONCLUSION: Based on the patients' reports, it was possible to conclude that there were no statistic differences between both groups, that is, the enlargement of the apical foramen did not influenced in the increase of pain.
