Double teeth are an anomalous dental formation resulting from the union of two dental germs or their attempt to separate. Fusion is characterized by the union of two adjacent teeth, while the gemination characterized by the attempt to divide a single tooth. This anomaly can occur in deciduous and permanent teeth. However, there is a higher prevalence in deciduous teeth, usually unilateral and involving one or two adjacent teeth in the region of lower incisors and canines. Objective: The objective of this study is to describe the treatment of a typical case of double tooth in posterior primary dentition. Case report: This article reports an unusual case of double tooth in a thirty-four-months old patient resulting from the fusion of a canine with the first upper deciduous molar. Pulp necrosis was observed only in one of the teeth involved on the fusion that was treated endodontically with an iodoform-based paste. The clinical and radiographic success of the case was confirmed by the clinical follow up of the patient. Final considerations: The dentist should be able to the conduct the correct diagnosis and ready attendance of the anomalies of the odontogenesis, being capable to accomplish the most appropriate approach for each observed clinical condition.
