Objective: The objective of this study was to review the scientific literature on Bisphophonates (BPs), its indications and contraindications, mechanism of actions and side effects that may occur in patients who are submitted to this therapy and verify the influence of the LLT (low level laser therapy) in the treatment of osteonecrosis. Review of literature: the BPs inhibit the osteoclastic activity and induce osteoclast apoptosis. These drugs are used in the treatment of metastatic bone disease and osteoporosis, malignant neoplasms with bone metastasis, malignant hypercalcemia and multiple mieloma. The chronic use of bisphosphonates may cause osteonecrosis of jaws after dental treatment. When bone necrosis occurs, the LLLT has been considered an alternative of treatment, improving clinical and symptomatologic condition. Final considerations: The treatment of this bone necrosis is difficult and long. Before these systemic alterations observed with the use of this medicament, dentistry has also searched for indications for its use. New lines of research have been developed in areas as implantology and endodontics, using the BPs as a medicament of topical use.
