Introduction: Deciduous teeth maintain space, in mesial-distal and cervical-occlusal dimentions for the adequate positioning of permanent teeth. When premature loss of deciduous teeth occurs, adjacent teeth may migrate to the region, leading to: closure or reduction of the space designed to the eruption of the permanent successor tooth, arch curtailment and antagonist tooth extrusion. Keeping the length of the dental arch for the eruption of permanent teeth, when there is premature loss, is an activity of great importance for malocclusions prevention. When bilateral premature loss of one or more mandibular molars occurs, the fixed lingual arch is chosen as the maintaining space. The objective of this study is to describe the clinical and laboratorial confection phases of fixed Lingual Arch, by using a case report as an example. Case Report: A mixed dentition patient who had orthodontic treatment at the clinic of the Orthodontics Department of the Dentistry Faculty of UFRGS, using the lingual arch as maintaining space and brackets bonded in the first pre-molars, and wire segment and open springs between them and the first permanent molars, to allow the eruption of the second lower pre-molars. Conclusion: The treatment results may be considered as satisfactory, especially if we consider the simplicity of the mechanical components used. The lingual arch has a great influence in the prevention of malocclusion in cases of premature teeth loss, since it prevents the lingual movement of permanent lower incisors and the mesial movement of first permanent molars.
