Objective: To investigate the frequency of submission of the terms of clarification, and to verify the main complications observed in endodontic treatments of patients seen by an accredited Dental Health Plan Operator. Methods: Cross-sectional observational study that conducted a retrospective data survey in documents called terms of clarification of a large operator in the years 2010 and 2011. The quoted term is sent for audit whenever the provider dentist verifies any complications in the endodontic treatment and aims to make the patient aware of what happened. Results: 61.40% of the terms analyzed were female, and 38.60% male. The relative frequency between the term sent and the procedure performed ranged between 1.90% and 8.46%. There were a total of 13,968 endodontic treatments performed, and only 702 documents sent. The most common complications were atresia/calcification of the conduit (204), followed by the overfilling (129) and filling beyond the root apex (124). Conclusion: The frequency of notification of the Term of Clarification by the ones accredited to the Dental Health Plan Operator was low. The most common complications reported were: Atresia/ calcification of the conduit, overfilling and filling beyond the root apex.
