Objective: to evaluate the current predictability of short implants and extra-short not splinted in the jaw and further discuss issues related to their classifications. Literature review: the rehabilitation of atrophic posterior areas presents major complexity in the treatment planning and therapy with dental implants. Augmentation surgeries of alveolar areas defects have no conclusive predictability in the literature, and then implants with reduced length form has been the issue of several studies. The use of short implants decreases morbidity, operative time, treatment duration, surgical complications and medication use compared to prior rehabilitation with bone grafts. Final considerations: there is a need for standardization of studies (human clinical trials), to establish rates of success / survival appropriate to the peculiarities of short implants, aiming to promote definitions based in the predictability of these implants. Clinical trials with adequate study design, providing more reliable results are essential for clinical approaches.
