Prior literature describes the oroantral fistula as a direct access, covered by an epithelial tissue, between the maxillary sinus and the oral cavity, which occurs accidentally during a dental extraction when the apical third is near the sinus cavity. The diagnosis involves clinical and radiographic procedures, and the Valsalva's Maneuver is an important step in the physical examination. The surgical technique to close these fistulas is still under discussion within the literature, but the use of the buccal fat pad as a pedicullate graft to close intraoral defects has gained recognition, as it is a quick and relatively easy surgical procedure with a high success rate. This article describes an oroantral fistula case which employed the buccal pedicullate flap as the main surgical treatment. After a 30-day follow-up period, the patient reported being satisfied and presented no remaining signs nor symptoms, a total closure of the oroantral fistula, as well as a lack of fat tissue and the presence of oral epithelial tissue.
