Osteosarcoma is an aggressive malignancy of mesenchymal origin, characterized by irregular immature bone formation, osteoid matrix production and malignant spindle cell stroma. It is the most common primary malignant tumor of bone, accounting for approximately 20% of sarcomas, but only 5% of these occur in the jaw. The most common symptoms are local swelling, pain and functional limitation. The etiologic factors are associated with patient characteristics (age, sex, race, ethnicity, weight, height) genetic factors and pre-existing bone abnormalities. Diagnosis is obtained through CT scans and pathology. The current treatment for osteosarcoma consists of surgical resection and additional chemotherapy. The prognosis of these patients has improved in recent years due to proper staging of the lesions, accurate imaging methods and especially multidrug therapy. This article describes a case of Osteosarcoma in an individual of 39 years, female, with a large exophytic lesion of the right mandibular body, facial asymmetry and evolution of three months. The diagnosis was established by clinical, imaging and histopathology. We conclude that surgical treatment is crucial early primary lesion to minimize recurrent and metastatic behavior.
