The odontogenic myxoma is a tumor that microscopically mimics the dental pulp or the follicular sac. Despite being a benign tumor, it is aggressive and may recur due to the absence of the capsule and its soft consistency. The optimum treatment is radical as curettage may result in incomplete removal. Despite the potential for recurrence, the prognosis is good. A 27-year-old female patient came to us for treatment with a large painless swelling in the left mandibular body, causing displacement of bicuspids and molars and cortical bone expansion. The patient had been treated for a year with antibiotics by another dentist but without success. By the time she came to us she had occlusion problems. In the X-ray evaluation, there was a unilocular radiolucent image, with the lower border at the base of the mandible. The result of histopathology was an odontogenic myxoma. Treatment included radical bone resection from the midline to the ipsilateral mandibular ramus, preserving the condyle and the coronoid process. The reconstruction was made using a 2.4 unilock titanium plate at the same time as the resection. Histopathology was again positive for an odontogenic myxoma with free margins. After fifteen months of clinical and radiographic follow-up, there have been no recurrences. We are currently waiting to refer her for a microvascularized bone graft reconstruction
