Management of temporomandibular joint bony ankylosis as sequel of mandibular trauma in pediatric patient. Rev. cir. traumatol. buco-maxilo-fac. [online]. 2014, vol.14, n.3, pp. 53-58. ISSN 1808-5210.
Ankylosis of the temporomandibular joint (TMJ) refers to the fusion of the complex intracapsular disc-condyle and the articular fossa of the temporal bone, with restriction of joint movement and consequent limitation of mouth opening, restricted masticatory capacity, difficulty in speech, and poor oral hygiene, in addition to psychological disorders such as difficulty with social interaction. This study aims to report a case of unilateral TMJ ankylosis. A 9-year-old female patient and discuss the issues involved in treatment by means of interpositional arthroplasty with costochondral graft and muscle flap temporal reconstructive surgery on the TMJ. We conclude that treatment using interpositional arthroplasty with temporalis muscle flap and costochondral graft is an excellent alternative for treating patients with TMJ ankylosis during growth because the costochondral graft has morphological characteristics similar to the mandibular condyle growth potential, which allows the graft to track the growth spurt in the case of pediatric patients. The satisfactory outcome of treatment should be attributed to the correct choice of surgical technique and to follow-up including rigorous physical therapy and speech therapy.
