The removal of impacted third molars is a common surgical procedure that may be performed by general dentists and oral and maxillofacial surgeons alike. Tooth displacement into contiguous fascial spaces, however, is a rather uncommon surgical mishap that is associated with inadequate clinical and radiographic examination, lack of sound anatomical knowledge, inadequate surgical access, limited visibility and excessive or uncontrolled surgical movements. While the removal of a displaced tooth is normally performed after thorough clinical and radiographic assessments in a mediate fashion, immediate surgical rescue is also a possibility. The aim of this paper is to describe the management of an upper third molar accidentally displaced into the buccal space after an attempt to extract.
