Due to the needs of aesthetic, many restorations have margins with subgingival extensions. However, in order to perform an aesthetic work, a number of factors should be considered not only for the purpose of obtaining the aesthetic beauty but also the health of the supportive tissues. The objective of this work was to discuss the relationship between the location of the preparation margins and periodontal tissue response in aesthetic restorations. The methodology used was the evaluation of relevant articles selected from the PubMed database, in addition to the manual search. After examining the articles it could be concluded that: 1 - the supragengival margin is the best location in terms of periodontal health, 2 - restorations with subgengival margins caused damage to the periodontium, 3 - patients with proper oral hygiene did not show reduction in periodontal damage caused by restorations with subgengival margins; 4 - The location of the preparation margins in aesthetic restorations may be extended subgingivally, provided that the supracrestal gingival tissue measurements are verified and respected contralaterally and individually.
