O objetivo deste estudo foi comparar o diâmetro do preparo adquirido pelo instrumento reciprocante Wave One Primary (Dentsply Maillefer, Ballaigues, VD, Switzerland) e o diâmetro de cones de guta-percha Wave One Primary. Dezesseis canais mesiais de molares inferiores foram instrumentados com a lima reciprocante Wave One Primary, e os canais moldados com resina acrílica. Após descalcificação do dente, a resina foi removida e analisada num perfilógrafo (Werth GmbH, Giessen, HE, Alemanha). Dezesseis cones de guta-percha Wave One Primary também foram analisados. Os diâmetros do terço cervical, médio e apical dos preparos (resina) e dos cones de guta-percha foram submetidos ao teste U de Mann Whitney (p=0.05). Os dados apresentaram diferença significativa em todos os terços com preparos mais amplos que os cones de guta-percha. Pode-se concluir que o diâmetro do preparo produzido pelas limas Wave One Primary não possuem correspondência ao seu respectivo cone de guta-percha, cujo preparo é mais amplo em todos os terços.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[The fit of gutta-percha cone on the root canal shaping is important to the step obturation. Therefore, the aim of this study was to compare the diameter of the shaping produced by Wave One Primary reciprocating single-file (Dentsply Maillefer, Ballaigues, VD, Switzerland) and the Wave One Primary gutta-percha points. Sixteen mesial root canals of mandibular molars were prepared using Wave One Primary reciprocating single-file, and then they were filled with acrylic resin. After decalcification of teeth, the resin was removed and analyzed using perfilograph (Werth GmbH, Giessen, HE, Germany). Sixteen Wave One Primary gutta-percha points were also analyzed. The diameter of third coronal, medium and apical of the shaping (resin) and gutta-percha points were compared using Mann Whitney U test (p=0.05). The data showed significant difference in all thirds with shaping larger than gutta-percha points. It can conclude that the diameter of shaping produced by Wave One Primary reciprocating single-file does not match the diameter of its gutta-percha point, the shaping is larger in all thirds.]]></p></abstract>

No entanto, a correta adapta&ccedil;&atilde;o do cone principal ao preparo p&oacute;s-instrumenta&ccedil;&atilde;o &eacute; importante para o selamento do sistema de canais radiculares.</font></p>     <p>&nbsp;</p>     <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><B> INTRODU&Ccedil;&Atilde;O</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Durante as &uacute;ltimas d&eacute;cadas, a Endodontia vem recebendo   in&uacute;meros avan&ccedil;os e aplica&ccedil;&otilde;es de recursos tecnol&oacute;gicos com a   proposta de aumentar o progn&oacute;stico positivo do tratamento,   contribuindo cada vez mais com a preserva&ccedil;&atilde;o dos dentes na cavidade oral.</font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Assim, entre estes novos recursos, pode-se destacar a instrumenta&ccedil;&atilde;o   automatizada rotat&oacute;ria com instrumentos de n&iacute;quel   tit&acirc;nio e a obtura&ccedil;&atilde;o com cone &uacute;nico, ressaltando qualidade do   preparo mais uniforme e c&ocirc;nico<sup>1,2</sup> e menor tempo de trabalho<sup>3</sup>,   reduzindo a fadiga profissional e o estresse causado ao paciente.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Neste contexto, foi lan&ccedil;ado em 2010, um novo conceito de   instrumento automatizado, que utiliza o movimento reciprocante,   exemplificado pelo sistema Wave One (Dentsply Maillefer, Ballaigues,   VD, Switzerland). Tal movimento &eacute; gerado por um motor   el&eacute;trico que resulta no avan&ccedil;o da lima ao conduto por um &acirc;ngulo   rotacional de maior amplitude no sentido hor&aacute;rio, e o corte da   dentina num menor &acirc;ngulo rotacional no sentido anti-hor&aacute;rio.   Al&eacute;m disso, este sistema possui um instrumento de caracter&iacute;sticas   peculiares, reduzindo o n&uacute;mero de limas para todo o preparo, pois   a proposta &eacute; realizar o preparo com somente uma &uacute;nica lima e,   consequentemente, menor tempo de trabalho<sup>4,5</sup> e melhor resist&ecirc;ncia   do instrumento<sup>6</sup>. Ademais, cones de guta-percha com o perfil   da lima Wave One tamb&eacute;m foram lan&ccedil;ados para a realiza&ccedil;&atilde;o da   obtura&ccedil;&atilde;o com a t&eacute;cnica do cone &uacute;nico.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  A t&eacute;cnica de cone &uacute;nico tem uma proposta bastante interessante   no sentido de diminuir o tempo de trabalho e o n&iacute;vel   de dificuldade para sua realiza&ccedil;&atilde;o. No entanto, a t&eacute;cnica original  &eacute; realizada com o cone de guta-percha referente &agrave;s mesmas   caracter&iacute;sticas de conicidade e di&acirc;metro do &uacute;ltimo instrumento   utilizado. Neste particular, Machado (2007)<sup>7</sup> preconiza o uso do   cone principal de maior calibre que o &uacute;ltimo instrumento ap&oacute;s   o preparo mecanizado com sistema ProTaper (Dentsply Maillefer,   Ballaigues, VD, Switzerland), caracterizando uma t&eacute;cnica de cone  &uacute;nico modificada. Esta modifica&ccedil;&atilde;o se justifica pelo fato de que o   preparo finalizado com ProTaper possui maiores desgastes que a  &uacute;ltima lima utilizada.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Neste aspecto, o sistema Wave One ainda n&atilde;o foi analisado,   tornando-se importante verificar a correspond&ecirc;ncia de adapta&ccedil;&atilde;o   de seu cone de obtura&ccedil;&atilde;o ao di&acirc;metro do preparo produzido pela   lima. Al&eacute;m disso, como toda nova t&eacute;cnica, &eacute; necess&aacute;ria contesta&ccedil;&atilde;o   cient&iacute;fica justificando a elabora&ccedil;&atilde;o de ensaios que possam   estabelecer um paralelo qualitativo, buscando procedimentos   mais simples e eficientes que, sem d&uacute;vida, ir&atilde;o colaborar de sobremaneira   no sucesso da Endodontia e, ainda, ser replicada por   um n&uacute;mero maior de profissionais. Assim, o objetivo do presente   estudo foi comparar o di&acirc;metro do preparo adquirido pelo instrumento   reciprocante Wave One Primary e seu respectivo cone de   guta-percha.</font></p>     <p>&nbsp;</p>     <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><B>MATERIAIS E M&Eacute;TODOS</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Ap&oacute;s a aprova&ccedil;&atilde;o pelo Comit&ecirc; de &Eacute;tica e Pesquisa da Faculdade de Odontologia da Universidade de S&atilde;o Paulo (46/11), foram utilizadas para este estudo oito ra&iacute;zes mesiais de molares inferiores com dois condutos distintos, sem obstru&ccedil;&atilde;o e separadas da raiz distal. </font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B>Preparo dos condutos</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Os condutos (m&eacute;sio vestibular e m&eacute;sio lingual de todos os dentes, totalizando n=16) foram explorados com uma lima K10 (Dentsply Maillefer, Ballaigues, VD, Switzerland) e a odontometria realizada com recuo de 1mm do comprimento real do canal. A instrumenta&ccedil;&atilde;o manual at&eacute; a lima K15 (Dentsply Maillefer, Ballaigues, VD, Switzerland) foi feita com irriga&ccedil;&atilde;o com 2 mL de hipoclorito de s&oacute;dio 1% (F&oacute;rmula e a&ccedil;&atilde;o, S&atilde;o Paulo, Brasil) associado ao Endo PTC leve (F&oacute;rmula e A&ccedil;&atilde;o, S&atilde;o Paulo, SP, Brasil). Ap&oacute;s a instrumenta&ccedil;&atilde;o, os canais foram instrumentados com a lima Wave One Primary (Dentsply Maillefer, Ballaigues, VD, Switzerland) que possui di&acirc;metro apical inicial de 25.08: com o motor em movimento rec&iacute;proco (Dentsply Maillefer, Ballaigues, VD, Switzerland) penetrando suavemente no ter&ccedil;o cervical e retirando, depois no m&eacute;dio e seguidamente no cervical at&eacute; o comprimento de trabalho. A cada introdu&ccedil;&atilde;o da lima ao canal foi realizada irriga&ccedil;&atilde;o com 2 mL de hipoclorito de s&oacute;dio 1% totalizando 6 mL no preparo, e irriga&ccedil;&atilde;o final com 5 mL de hipoclorito de s&oacute;dio 1%, seguido de 5 mL de EDTA 17% (F&oacute;rmula e A&ccedil;&atilde;o, S&atilde;o Paulo, SP, Brasil) e mais 5 mL de hipoclorito de s&oacute;dio 1%. Os canais foram secos por aspira&ccedil;&atilde;o e com pontas de papel Wave One Primary (Dentsply Maillefer, Ballaigues, VD, Switzerland).</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B>Modelagem dos canais para obten&ccedil;&atilde;o da forma do preparo</B></font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Os canais foram preenchidos com resina vermelha Duralay (Reliance, Worth, IL, EUA) na sua fase arenosa com aux&iacute;lio do instrumento propulsor L&ecirc;ntullo 25 (Dentsply Maillefer, Ballaigues, VD, Switzerland) em baixa rota&ccedil;&atilde;o diminu&iacute;da. Ap&oacute;s a polimeriza&ccedil;&atilde;o total, os dentes foram envolvidos por uma gaze e imersos primeiramente em &aacute;cido f&oacute;rmico 10% por 2 dias e, posteriormente, em &aacute;cido f&oacute;rmico 20% por mais 7 dias. Com aux&iacute;lio de uma l&acirc;mina de bisturi, as amostras moldadas em resina foram cuidadosamente retiradas do remanescente dent&aacute;rio e, assim, lavadas em &aacute;gua corrente para posterior an&aacute;lise.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B>An&aacute;lise das amostras do preparo</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Cada amostra foi acomodada sob a plataforma de um perfil&oacute;grafo (Werth GmbH, Giessen, HE, Alemanha), no qual, atrav&eacute;s de proje&ccedil;&atilde;o por luz com aumento de 20x, foi medido o calibre das amostras (<a href="#fig01">Figura 1</a>). Atrav&eacute;s do eixo horizontal do perfil&oacute;grafo foi ajustada a altura para ser analisada, no qual se optou pelo D0 (0mm da ponta) referindo-se ao t&eacute;rmino de ter&ccedil;o apical, pelo D3 (3mm da ponta) referindo-se ao t&eacute;rmino do ter&ccedil;o m&eacute;dio, e pelo D5 (5mm da ponta) referindo-se ao t&eacute;rmino do ter&ccedil;o cervical. Depois de posicionada a altura, o di&acirc;metro foi medido utilizando o eixo vertical do perfil&oacute;grafo. </font></p>      <p>&nbsp;</p>     <p><a name="fig01"></a></p>     <p>&nbsp; </p>     <p align="center"><img src="/img/revistas/apcd/v67n2/a12fig01.jpg">     <p>&nbsp;</p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B>An&aacute;lise dos cones de guta percha Wave One Primary</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Dezesseis cones de guta-percha Wave One Primary (Dentsply Maillefer, Ballaigues, VD, Switzerland) foram utilizados. Os cones foram posicionados e medidos da mesma maneira que as amostras do preparo. Contudo, 0,07mm foram descontados no posicionamento do D0, j&aacute; que o t&eacute;rmino da ponta do cone &eacute; arredondado (<a href="#fig02">Figura 2</a>). </font></p>      ]]></body>

<body><![CDATA[<p align="center"><img src="/img/revistas/apcd/v67n2/a12tab01.jpg">     <p>&nbsp;</p>     <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><B>RESULTADOS</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">De acordo com os di&acirc;metros medidos, o preparo p&oacute;s-instrumenta&ccedil;&atilde;o com Wave One Primary obteve significantemente maiores calibres em todos os ter&ccedil;os quando comparado aos cones de guta-percha (p&lt;0.0001), apresentando preparo mais amplo que o volume do cone. Os valores m&eacute;dios de cada ter&ccedil;o podem ser visualizados na <a href="#tab01">tabela 1</a>.</font> </p>    <p>&nbsp;</p>     <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><B> DISCUSS&Atilde;O</B></font></p>      <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">A correta adapta&ccedil;&atilde;o do cone de guta-percha &agrave; parede do canal p&oacute;s-preparo &eacute; de suma import&acirc;ncia, uma vez que o selamento do sistema de canais radiculares &eacute; dependente do preenchimento com material obturador. A propor&ccedil;&atilde;o da quantidade entre cimento obturador e guta-percha &eacute; discutida, por&eacute;m, o que pode ser observado na literatura &eacute; uma prefer&ecirc;ncia entre a guta-percha com rela&ccedil;&atilde;o ao cimento   utilizado<sup>8-11</sup>. Tais prefer&ecirc;ncias est&atilde;o baseadas nas limita&ccedil;&otilde;es   destes compostos que poder&atilde;o sofrer processos de solubilidade,   comprometendo, assim, a pr&oacute;pria finalidade desta etapa   da t&eacute;cnica endodontica<sup>12</sup>. Neste particular, Machado (2007)<sup>7</sup>   estabelece que os preparos originados por sistemas mecanizados   em rota&ccedil;&atilde;o cont&iacute;nua com ProTaper n&atilde;o correspondem   exatamente &agrave; &uacute;ltima utilizada, e que os preparos ser&atilde;o mais   amplos que tais instrumentos. Fato este que n&atilde;o havia sido   avaliado at&eacute; o presente momento diante dos sistemas reciprocantes.   A escolha pelo sistema Wave One se justifica devido   suas caracter&iacute;sticas de sec&ccedil;&atilde;o e conicidade semelhante ao   sistema ProTaper. No caso da Wave One Primary, com semelhan&ccedil;a  &agrave; ProTaper F2. A escolha espec&iacute;fica &agrave; Primary se deve  &agrave; informa&ccedil;&atilde;o de que &eacute; a lima indicada para maior parte dos casos na cl&iacute;nica di&aacute;ria.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Ainda no que se concerne ao modelo metodol&oacute;gico, utilizou-se ra&iacute;zes mesiais de molares inferiores, pois s&atilde;o provenientes   de um grupo dental (molares) com alta incid&ecirc;ncia de   tratamentos endod&ocirc;nticos; e esta raiz, em particular, devido&agrave;  presen&ccedil;a de dois canais distintos e com menor varia&ccedil;&atilde;o anat&ocirc;mica   quanto &agrave; forma, viabilizando a padroniza&ccedil;&atilde;o dos esp&eacute;cimes.   Al&eacute;m disso, a caracter&iacute;stica de um canal pouco amplo&eacute;  importante devido ao desgaste limitado das limas Wave One   Primary (25.08), que podem ser utilizadas somente em canais   explor&aacute;veis at&eacute; a lima<sup>15</sup>.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Os resultados obtidos neste estudo apresentaram que o   preparo produzido pela Wave One Primary n&atilde;o corresponde &agrave;  forma do cone de guta-percha de seu sistema, sendo o preparo   mais amplo que a forma esperada pelo fabricante. Estes   resultados parecem estar de acordo com o mesmo comportamento   obtido pelas limas ProTaper em rota&ccedil;&atilde;o cont&iacute;nua que   permitem a utiliza&ccedil;&atilde;o de um cone de maior calibre que o &uacute;ltimo   instrumento<sup>7,9,10,13</sup>. Possivelmente, a semelhan&ccedil;a destes   resultados deve-se &agrave; sec&ccedil;&atilde;o semelhante destes dois sistemas,   que possuem &acirc;ngulos de corte agressivos, promovendo grande   corte de dentina.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  A m&eacute;dia do calibre obtido nos cones de guta-percha em   D0 foi de 0,25 mm, no qual segue &agrave;s especifica&ccedil;&otilde;es do Wave   One Primary referidas pelo fabricante (D0=0,25 mm). Estes   resultados contradizem alguns estudos que t&ecirc;m apresentado   que cones de guta-percha podem estar descalibrados<sup>14-16</sup>,   e, neste &acirc;mbito, os cones de guta-percha Wave One Primary   podem ser referenciais de compara&ccedil;&atilde;o &agrave;s amostras obtidas p&oacute;s-preparo utilizadas neste estudo.</font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">&Eacute; importante ressaltar que estes novos sistemas podem colaborar   com a qualidade, tempo e facilidade na execu&ccedil;&atilde;o da terapia   endod&ocirc;ntica. Contudo, aten&ccedil;&atilde;o especial deve ser tomada quando   da obtura&ccedil;&atilde;o destes condutos pela t&eacute;cnica de cone &uacute;nico, onde   mais estudos dever&atilde;o ser realizados para complementar a aplicabilidade cl&iacute;nica da t&eacute;cnica modificada.</font></p>     <p>&nbsp;</p>     <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>CONCLUS&Otilde;ES</b> </font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">O di&acirc;metro do preparo produzido pela lima Wave One Primary n&atilde;o possue correspond&ecirc;ncia ao seu respectivo cone de guta-percha, cujo preparo &eacute; mais amplo em todos os ter&ccedil;os.</font><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> </font></p>     <p>&nbsp;</p>     <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>APLICA&Ccedil;&Atilde;O CL&Iacute;NICA</b> </font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Maior aten&ccedil;&atilde;o para a escolha do cone principal durante a obtura&ccedil;&atilde;o deve ser encorajada; cones de maior calibre que o Wave One Primary podem ser utilizados ap&oacute;s a instrumenta&ccedil;&atilde;o com seu respectivo sistema.</font></p>     <p>&nbsp;</p>     <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>AGRADECIMENTOS</b> </font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Trabalho extra&iacute;do da Inicia&ccedil;&atilde;o Cient&iacute;fica com bolsa financiada pelo CNPq, 2011/394.
