O sucesso do tratamento endodôntico depende do reconhecimento das idiossincrasias de cada uma destas três condições. Canais radiculares contendo polpa necrosada e lesão perirradicular representam um problema infeccioso e devem ser tratados de forma diferenciada dos dentes com polpa viva (sem infecção). O sucesso do tratamento endodôntico nestes casos vai depender do controle da infecção exercido pelo preparo químico-mecânico, medicação intracanal e obturação. Este artigo revisa os princípios biológicos do tratamento de dentes com polpa necrosada e infectada, visando fornecer ao clínico as condições para a melhora no resultado do tratamento endodôntico destes casos.]]></p></abstract>

Canals of necrotic teeth associated with apical periodontitis represent an infectious problem and should be treated differently from vital teeth (non-infected). The success of the endodontic treatment will depend on effective infection control promoted by chemomechanical preparation, intracanal medication and obturation. <body><![CDATA[<p>&nbsp;</p>     <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><B> Introdu&ccedil;&atilde;o</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">As tr&ecirc;s condi&ccedil;&otilde;es cl&iacute;nicas que o profissional lida no dia a dia da pr&aacute;tica   endod&ocirc;ntica e que requerem interven&ccedil;&atilde;o incluem dentes com pulpite   irrevers&iacute;vel (biopulpectomia), com necrose e infec&ccedil;&atilde;o pulpar (necropulpectomia)   e casos de retratamento <sup>26</sup>. O sucesso do tratamento endod&ocirc;ntico   depende do reconhecimento das idiossincrasias de cada uma destas   tr&ecirc;s condi&ccedil;&otilde;es. A diferen&ccedil;a fundamental entre elas reside no fato de que os   casos de polpa necrosada e de retratamento s&atilde;o caracterizados pela presen&ccedil;a   de infec&ccedil;&atilde;o, enquanto os de polpas vitais s&atilde;o livres de infec&ccedil;&atilde;o. Para que um   elevado &iacute;ndice de sucesso de magnitude similar seja obtido para estas tr&ecirc;s   condi&ccedil;&otilde;es, deve-se reconhecer que medidas terap&ecirc;uticas diferenciadas devem   ser institu&iacute;das. Em outras palavras, o tratamento para tais condi&ccedil;&otilde;es deve ser   calcado em estrat&eacute;gias diferentes, se o mesmo &iacute;ndice de sucesso &eacute; esperado.   Em outro artigo discutimos os princ&iacute;pios biol&oacute;gicos do tratamento endod&ocirc;ntico   de dentes com polpa viva. O presente artigo discute os aspectos filos&oacute;ficos   relacionados ao tratamento endod&ocirc;ntico de dentes com polpa necrosada e les&atilde;o perirradicular associada.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B> Infec&ccedil;&otilde;es Endod&ocirc;nticas</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">As les&otilde;es perirradiculares s&atilde;o doen&ccedil;as infecciosas causadas por micro-organismos infectando o sistema de canais radiculares <sup>26</sup> (<a href="#fig01">Figura 1</a>). Na   necropulpectomia, o endodontista lida com a infec&ccedil;&atilde;o prim&aacute;ria do canal,   enquanto que casos de retratamento est&atilde;o relacionados a uma infec&ccedil;&atilde;o persistente ou secund&aacute;ria do canal.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Mais de 400 esp&eacute;cies bacterianas diferentes t&ecirc;m sido detectadas em canais   radiculares infectados, usualmente em combina&ccedil;&otilde;es de 10 a 30 esp&eacute;cies   na infec&ccedil;&atilde;o prim&aacute;ria, com grande preval&ecirc;ncia de esp&eacute;cies anaer&oacute;bias   estritas <sup>26</sup>. Estudos empregando m&eacute;todos de cultura para anaer&oacute;bios ou   metodologia avan&ccedil;ada de biologia molecular t&ecirc;m revelado que os g&ecirc;neros de   bact&eacute;rias anaer&oacute;bias estritas mais prevalentes em infec&ccedil;&otilde;es prim&aacute;rias s&atilde;o   Treponema, Tannerella, Fusobacterium, Dialister, Prevotella, Porphyromonas,   Parvimonas, Peptostreptococcus, Pseudoramibacter, Eubacterium e Actinomyces <sup>36</sup>.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  As infec&ccedil;&otilde;es endod&ocirc;nticas s&atilde;o causadas por biofilmes bacterianos intrarradiculares   <sup>17</sup>. Na infec&ccedil;&atilde;o prim&aacute;ria, muitas c&eacute;lulas bacterianas encontram-se em suspens&atilde;o nos fluidos presentes na luz do canal principal, mas   agregados bacterianos na forma de biofilmes s&atilde;o usualmente visualizados   aderidos &agrave;s paredes dentin&aacute;rias do canal <sup>17</sup> (<a href="#fig02">Figura 2</a>). A infec&ccedil;&atilde;o pode se   propagar para t&uacute;bulos dentin&aacute;rios e para ramifica&ccedil;&otilde;es apicais, canais laterais, istmos e t&uacute;bulos dentin&aacute;rios <sup>36</sup>. </font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B> Objetivos da Necropulpectomia</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Conceitualmente, as doen&ccedil;as infecciosas (como as les&otilde;es perirradiculares)   s&atilde;o tratadas atrav&eacute;s da elimina&ccedil;&atilde;o dos micro-organismos causadores.&Eacute;  neste contexto que se insere o tratamento de dentes com polpa necrosada,   ou seja, al&eacute;m da import&acirc;ncia de se prevenir a introdu&ccedil;&atilde;o de novos micro-organismos no interior do sistema de canais radiculares, deve-se eliminar a infec&ccedil;&atilde;o endod&ocirc;ntica ou reduzi-la significativamente para que o tratamento logre &ecirc;xito. Assim, prevenir ou tratar a infec&ccedil;&atilde;o endod&ocirc;ntica &eacute; o principal objetivo do profissional que pratica a Endodontia.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Para efeito de tratamento, deve-se considerar todo dente   contendo polpa necrosada como infectado, independentemente   da detec&ccedil;&atilde;o radiogr&aacute;fica de uma les&atilde;o perirradicular.   Em alguns casos de dentes com polpa necrosada sem les&atilde;o   perirradicular diagnosticada radiograficamente, o aparecimento   da les&atilde;o &eacute; apenas uma quest&atilde;o de tempo. Em outros,   a les&atilde;o pode estar presente, mas ainda n&atilde;o acarretou em destrui&ccedil;&atilde;o  &oacute;ssea suficiente para que fosse discernida radiograficamente   <sup>4</sup>. Assim, o tratamento destes casos de dentes com   polpa necrosada sem les&atilde;o perirradicular aparente dever&aacute;  ser o mesmo dos dentes onde a les&atilde;o &eacute; vis&iacute;vel na radiografia.   Reconhecidas as diferen&ccedil;as essenciais de infec&ccedil;&atilde;o entre   as tr&ecirc;s condi&ccedil;&otilde;es cl&iacute;nicas citadas anteriormente, recomenda-se: a) sess&atilde;o &uacute;nica para dentes com polpa viva; b) duas   sess&otilde;es com pasta de hidr&oacute;xido de c&aacute;lcio em ve&iacute;culo biologicamente   ativo nos casos infectados (necrose pulpar e retratamento).</font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B> Controle da Infec&ccedil;&atilde;o</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> A necessidade de se prevenir ou controlar a infec&ccedil;&atilde;o endod&ocirc;ntica,   visando ao reparo das estruturas perirradiculares   e ao restabelecimento da fun&ccedil;&atilde;o dent&aacute;ria normal e da   sa&uacute;de bucal, forma a base s&oacute;lida na qual se fundamenta a   Endodontia Contempor&acirc;nea. O tratamento endod&ocirc;ntico   apresenta tr&ecirc;s etapas principais de controle da infec&ccedil;&atilde;o: o   preparo qu&iacute;mico-mec&acirc;nico, a medica&ccedil;&atilde;o intracanal e a obtura&ccedil;&atilde;o do sistema de canais radiculares <sup>23</sup>. </font></p>      <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B> Efeito do Preparo Qu&iacute;mico-Mec&acirc;nico</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Durante o preparo qu&iacute;mico-mec&acirc;nico, instrumentos   endod&ocirc;nticos promovem a remo&ccedil;&atilde;o mec&acirc;nica de micro-organismos, seus produtos e tecidos degenerados, auxiliadas   por uma subst&acirc;ncia qu&iacute;mica que, al&eacute;m de maximizar   a remo&ccedil;&atilde;o de detritos atrav&eacute;s da a&ccedil;&atilde;o mec&acirc;nica do   fluxo e refluxo, tamb&eacute;m pode exercer um efeito qu&iacute;mico   significativo, desde que possua a&ccedil;&atilde;o antimicrobiana e solvente de mat&eacute;ria org&acirc;nica <sup>27</sup>.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  A a&ccedil;&atilde;o mec&acirc;nica da instrumenta&ccedil;&atilde;o e da irriga&ccedil;&atilde;o&eacute;  capaz de reduzir substancialmente a quantidade de micro-organismos e de tecido degenerado do interior do sistema   de canais radiculares <sup>32</sup>. Todavia, o emprego de solu&ccedil;&otilde;es   irrigadoras (subst&acirc;ncia qu&iacute;mica auxiliar) dotadas de atividade   antibacteriana aumenta significativamente a efic&aacute;cia do preparo em termos de controle da infec&ccedil;&atilde;o <sup>5,44</sup>.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  O NaOCl &eacute; a subst&acirc;ncia qu&iacute;mica auxiliar mais empregada   no tratamento endod&ocirc;ntico de dentes com necrose   pulpar, em concentra&ccedil;&atilde;o variando entre 0,5 a 6%. Estudos   demonstram que n&atilde;o h&aacute; diferen&ccedil;a significativa entre as diferentes   concentra&ccedil;&otilde;es de NaOCl em termos de redu&ccedil;&atilde;o bacteriana   intracanal <sup>6,37</sup> e capacidade solvente de mat&eacute;ria   org&acirc;nica <sup>3</sup>. Assim, trocas regulares da solu&ccedil;&atilde;o no canal e   irriga&ccedil;&otilde;es copiosas mant&ecirc;m as propriedades das solu&ccedil;&otilde;es no   interior do canal, compensando os efeitos da concentra&ccedil;&atilde;o.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B> Calibre Apical de Instrumenta&ccedil;&atilde;o</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Estudos t&ecirc;m revelado que o calibre apical do preparo   pode exercer influ&ecirc;ncia significativa no controle da infec&ccedil;&atilde;o.   SIQUEIRA et al. <sup>32</sup> revelaram que a cada troca sequencial   de instrumentos para um maior calibre, a redu&ccedil;&atilde;o da popula&ccedil;&atilde;o   bacteriana foi significativamente maior quando comparada   com a lima anterior. Tal achado indica que quanto   mais amplo for o preparo do canal, maior tamb&eacute;m ser&aacute; a   elimina&ccedil;&atilde;o de bact&eacute;rias do seu interior, o que foi corroborado por ROLLISON et al. <sup>20</sup>.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Na pr&aacute;tica cl&iacute;nica, o di&acirc;metro final do preparo do canal   depender&aacute; do tipo de instrumento utilizado no preparo, do   volume radicular e da presen&ccedil;a de curvaturas. Instrumentos   rotat&oacute;rios e manuais confeccionados a partir de uma liga de   NiTi podem alargar canais curvos a di&acirc;metros dificilmente   alcan&ccedil;ados por instrumentos de a&ccedil;o inoxid&aacute;vel, com um   risco muito menor de acidentes transoperat&oacute;rios. Preparos   suficientemente amplos podem incorporar irregularidades   anat&ocirc;micas e permitir uma remo&ccedil;&atilde;o substancial de irritantes   do interior do sistema de canais radiculares <sup>24</sup>. Al&eacute;m   disso, preparos amplos permitem uma melhor irriga&ccedil;&atilde;o do   ter&ccedil;o apical dos canais <sup>1</sup>. </font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B> Momento da Obtura&ccedil;&atilde;o</B></font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">O tratamento endod&ocirc;ntico efetuado em sess&atilde;o &uacute;nica   apresenta algumas prov&aacute;veis vantagens para o profissional e   o paciente. Al&eacute;m de poupar tempo, previne a contamina&ccedil;&atilde;o   (dentes polpados) ou a recontamina&ccedil;&atilde;o (dentes despolpados)   que pode ocorrer entre as sess&otilde;es de tratamento. Em casos   de tratamento de dentes polpados (biopulpectomia), o tratamento   em sess&atilde;o &uacute;nica deve ser executado quando o fator   tempo, a habilidade do operador, as condi&ccedil;&otilde;es anat&ocirc;micas e   o material dispon&iacute;vel assim o permitirem. Por outro lado, a   obtura&ccedil;&atilde;o imediata em casos de dentes com polpa necrosada,   com uma infec&ccedil;&atilde;o endod&ocirc;ntica estabelecida e les&atilde;o perirradicular   associada ou n&atilde;o, representa ainda um motivo de controv&eacute;rsias entre os autores.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Dois fatores s&atilde;o cr&iacute;ticos quando se considera o tratamento   em sess&atilde;o &uacute;nica de dentes despolpados: a incid&ecirc;ncia de   sintomatologia p&oacute;s-operat&oacute;ria e o sucesso a longo prazo da   terapia. Observando a incid&ecirc;ncia de sensibilidade p&oacute;s-operat&oacute;ria   ap&oacute;s tratamentos endod&ocirc;nticos realizados em sess&atilde;o  &uacute;nica ou m&uacute;ltiplas, a maioria dos estudos atesta que n&atilde;o h&aacute;  diferen&ccedil;as significantes <sup>8,45</sup>. Entretanto, al&eacute;m da avalia&ccedil;&atilde;o   do desenvolvimento de sintomatologia ap&oacute;s o tratamento de   dentes despolpados em sess&atilde;o &uacute;nica, outro fator de extrema   relev&acirc;ncia deve ser levado em considera&ccedil;&atilde;o &ndash; o sucesso em   longo prazo do tratamento, ou seja, a capacidade do tratamento   de restabelecer a sa&uacute;de dos tecidos perirradiculares   por criar um ambiente prop&iacute;cio para a repara&ccedil;&atilde;o &oacute;ssea da les&atilde;o   perirradicular. Por ambiente prop&iacute;cio entende-se aquele   livre de micro-organismos e outros irritantes persistentes,   similar ao ambiente observado em casos de dentes polpados e capaz de levar ao sucesso em quase 100% dos casos. A busca   por um protocolo de tratamento que permite restabelecer   este ambiente favor&aacute;vel ao reparo perirradicular deve ser a   m&aacute;xima que norteia a Endodontia atual.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Na verdade, tanto em Medicina quanto na Odontologia,   o sucesso em longo prazo &eacute; o par&acirc;metro mais importante   pelo qual modalidades de tratamento s&atilde;o comparadas. Poucos   estudos bem controlados compararam o sucesso em   longo prazo da terapia endod&ocirc;ntica em dentes portadores de   necrose pulpar realizada em uma ou mais sess&otilde;es. Baseado   em alguns destes estudos, &eacute; poss&iacute;vel inferir que o tratamento   efetuado em uma ou mais sess&otilde;es utilizando uma pasta   de hidr&oacute;xido de c&aacute;lcio como medicamento oferece um &iacute;ndice   de sucesso 10 a 20% maior do que o efetuado em sess&atilde;o  &uacute;nica <sup>12,42,43,46</sup>. Entretanto, outros estudos mostram   praticamente aus&ecirc;ncia de diferen&ccedil;a percentual <sup>14</sup> ou mesmo   uma taxa de sucesso 10% maior com sess&atilde;o &uacute;nica.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  SATHORN et al. <sup>21</sup> tentaram realizar uma revis&atilde;o sistem&aacute;tica   da literatura lidando com este assunto e em 2005,   ano de publica&ccedil;&atilde;o do artigo, s&oacute; conseguiram incluir tr&ecirc;s   artigos que se qualificaram em seus crit&eacute;rios <sup>15,46,49</sup>.   Os dados para an&aacute;lise foram limitados e n&atilde;o permitiram   maiores conclus&otilde;es entre um m&eacute;todo e outro, havendo a necessidade   de muitos outros estudos com maior n&uacute;mero de   amostras para se alcan&ccedil;ar uma posi&ccedil;&atilde;o definida.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Em decorr&ecirc;ncia disto, enquanto estudos bem realizados   com elevado n&uacute;mero de amostras n&atilde;o forem disponibilizados,   o bom senso deve prevalecer no sentido de que se deve   optar por protocolos que previsivelmente eliminem a causa   das les&otilde;es perirradiculares. Em outras palavras, protocolos   com comprovada efic&aacute;cia antimicrobiana demonstrada por   estudos cl&iacute;nicos devem ser utilizados para o tratamento de   dentes com les&otilde;es perirradiculares. </font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B> Meta: Protocolo que Previsivelmente Proporcione Cultura Negativa</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Estudos cl&iacute;nicos demonstram que bact&eacute;rias sobrevivem   aos efeitos do preparo qu&iacute;mico-mec&acirc;nico em cerca de 40 a   60% dos casos, independentemente da solu&ccedil;&atilde;o irrigadora ou   da concentra&ccedil;&atilde;o empregada <sup>39</sup>. Muitas das bact&eacute;rias residuais   est&atilde;o destinadas a morrer, ou pela exposi&ccedil;&atilde;o a um   material obturador dotado de atividade antibacteriana ou   por serem confinadas ao interior do canal, ficando desprovidas   de nutrientes <sup>25</sup>. Contudo, em alguns casos, bact&eacute;rias   podem sobreviver mesmo a despeito de uma obtura&ccedil;&atilde;o   adequada do canal radicular, obtendo nutrientes e sobrevivendo   em n&uacute;mero suficiente para perpetuar uma les&atilde;o   perirradicular. Isto &eacute; claramente comprovado por estudos   demonstrando que &iacute;ndice de sucesso de dentes com canais   obturados na presen&ccedil;a de n&iacute;veis detect&aacute;veis de bact&eacute;rias cultiv&aacute;veis   (cultura positiva) &eacute; significativamente menor do que   em casos de cultura negativa <sup>42,47</sup>. Em outras palavras, a presen&ccedil;a de bact&eacute;rias no canal no momento da obtura&ccedil;&atilde;o&eacute; um importante fator de risco para o fracasso da terapia.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Uma vez que bact&eacute;rias remanescentes em localidades   anat&ocirc;micas inacess&iacute;veis aos instrumentos e &agrave; subst&acirc;ncia   qu&iacute;mica auxiliar representam um potencial para o fracasso   em longo prazo do tratamento endod&ocirc;ntico, medidas adicionais   que envolvam o controle deste processo infeccioso   devem ser empregadas durante a execu&ccedil;&atilde;o da terapia   em dentes despolpados. No presente momento, apenas   a medica&ccedil;&atilde;o intracanal com determinadas subst&acirc;ncias   qu&iacute;micas pode ser eficaz neste sentido. Assim, em casos   de necrose pulpar, o canal deve ser obturado em uma   segunda sess&atilde;o, ap&oacute;s a perman&ecirc;ncia de uma medica&ccedil;&atilde;o   intracanal antimicrobiana.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">&Eacute; imperioso salientar que a medica&ccedil;&atilde;o intracanal, assim   como qualquer outro procedimento, n&atilde;o ir&aacute; esterilizar   o canal. Embora este seja o ideal a ser alcan&ccedil;ado, o objetivo   vi&aacute;vel a ser atingido na realidade cl&iacute;nica &eacute; a m&aacute;xima   redu&ccedil;&atilde;o das popula&ccedil;&otilde;es bacterianas a um n&iacute;vel (limiar) que   seja compat&iacute;vel com o reparo perirradicular. Baseado em   estudos cl&iacute;nicos, este limiar pode ser interpretado como   o n&uacute;mero de c&eacute;lulas bacterianas aqu&eacute;m daquele necess&aacute;rio   para ser detectado pelo m&eacute;todo de cultura (cultura   negativa). Isto n&atilde;o significa que o cl&iacute;nico deva realizar   a cultura no consult&oacute;rio, mas sim que deve se basear na   literatura cient&iacute;fica para utilizar protocolos cl&iacute;nicos que   previsivelmente promovam um elevado &iacute;ndice de culturas   negativas antes da obtura&ccedil;&atilde;o.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B> Efeito da Medica&ccedil;&atilde;o Intracanal</B></font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Embora uma redu&ccedil;&atilde;o consider&aacute;vel no n&uacute;mero de c&eacute;lulas   bacterianas da luz do canal principal possa ser obtida   pelos efeitos qu&iacute;mico-mec&acirc;nicos da instrumenta&ccedil;&atilde;o e da   irriga&ccedil;&atilde;o, bact&eacute;rias podem permanecer vi&aacute;veis em regi&otilde;es   inacess&iacute;veis a estes. Enquanto menores irregularidades anat&ocirc;micas   possam ser incorporadas no preparo, &aacute;reas como   reentr&acirc;ncias, istmos, ramifica&ccedil;&otilde;es laterais e apicais e t&uacute;bulos   dentin&aacute;rios podem abrigar bact&eacute;rias que, uma vez n&atilde;o   eliminadas, p&otilde;em o resultado do tratamento em risco <sup>18</sup>.   Estas &aacute;reas n&atilde;o s&atilde;o comumente afetadas por instrumentos   e a subst&acirc;ncia qu&iacute;mica auxiliar empregada na irriga&ccedil;&atilde;o n&atilde;o   ter&aacute; tempo de a&ccedil;&atilde;o intracanal suficiente para agir em profundidade <sup>2,7</sup>.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Por permanecer por tempo mais prolongado no interior   do canal radicular, um medicamento intracanal dotado de   a&ccedil;&atilde;o antibacteriana tem maiores chances de atingir &aacute;reas   n&atilde;o afetadas pela instrumenta&ccedil;&atilde;o do canal. Assim, exercendo   sua a&ccedil;&atilde;o antibacteriana, pode contribuir decisivamente   para a m&aacute;xima redu&ccedil;&atilde;o da microbiota endod&ocirc;ntica. Por potencializar   esta redu&ccedil;&atilde;o, o emprego de curativos intracanais   est&aacute; diretamente relacionado a uma melhor repara&ccedil;&atilde;o dos   tecidos perirradiculares <sup>11,22</sup>.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Estudos cl&iacute;nicos demonstram que, em m&eacute;dia, 20 a 30%   dos canais ainda apresentam micro-organismos vi&aacute;veis ap&oacute;s   medica&ccedil;&atilde;o com hidr&oacute;xido de c&aacute;lcio em um ve&iacute;culo inerte   <sup>16,31</sup>. Al&eacute;m dos efeitos da dentina, de mat&eacute;ria org&acirc;nica e   de fluidos teciduais que podem tamponar o pH do hidr&oacute;xido   de c&aacute;lcio e assim reduzir sua efic&aacute;cia, alguns micro-organismos, como Enterococcus faecalis e Candida albicans,   comumente associados ao fracasso endod&ocirc;ntico <sup>19,35</sup>, s&atilde;o   resistentes ao efeito antimicrobiano pH-dependente do hidr&oacute;xido   de c&aacute;lcio <sup>28,48</sup>. Para compensar as defici&ecirc;ncias do   hidr&oacute;xido de c&aacute;lcio em ve&iacute;culo inerte, tem sido preconizada   a sua associa&ccedil;&atilde;o com outros agentes antimicrobianos, como   o paramonoclorofenol canforado (PMCC) e a clorexidina.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  V&aacute;rios estudos utilizando diferentes metodologias demonstram   que a pasta de hidr&oacute;xido de c&aacute;lcio com PMCC   apresenta excelente atividade antibacteriana e antif&uacute;ngica   <sup>28,29,38</sup>. Na verdade, a pasta de hidr&oacute;xido de c&aacute;lcio com   PMCC (e tamb&eacute;m glicerina, ou HPG) apresenta um excelente   raio de atua&ccedil;&atilde;o, amplo espectro de atividade antibacteriana,   rapidez na destrui&ccedil;&atilde;o de c&eacute;lulas bacterianas, retarda a   reinfec&ccedil;&atilde;o do canal quando da microinfiltra&ccedil;&atilde;o pelo selador   tempor&aacute;rio e &eacute; biocompat&iacute;vel, propriedades estas atestadas   por v&aacute;rios trabalhos <sup>9,22,28,29,33,38</sup>.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Outros medicamentos, como a clorexidina, tamb&eacute;m   apresentam um bom potencial para uso como medica&ccedil;&atilde;o   intracanal. Usada isoladamente ou associada ao hidr&oacute;xido   de c&aacute;lcio, a clorexidina atende aos requisitos de atividade   antimicrobiana satisfat&oacute;ria associada &agrave; baixa toxicidade <sup>10,41</sup>. Os bons resultados apresentados pela associa&ccedil;&atilde;o do hidr&oacute;xido   de c&aacute;lcio com a clorexidina (HCx) em v&aacute;rios estudos   cl&iacute;nicos recentes <sup>34,50</sup> permitem considerar esta pasta   como uma boa op&ccedil;&atilde;o de medica&ccedil;&atilde;o intracanal durante o   tratamento de dentes despolpados. </font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B> Efeito da Obtura&ccedil;&atilde;o</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Canais radiculares s&atilde;o usualmente obturados utilizando-se um material s&oacute;lido (geralmente a guta-percha e mais   recentemente o Resilon) associado com um pl&aacute;stico (os cimentos   endod&ocirc;nticos). Embora a guta-percha apresente   atividade antibacteriana, atribu&iacute;da ao componente de &oacute;xido   de zinco dos cones <sup>13</sup>, tal atividade &eacute; discreta e pouco   prov&aacute;vel de ter algum valor no interior do sistema de canais   radiculares. A maioria dos cimentos endod&ocirc;nticos apresenta   atividade antimicrobiana antes de endurecer, mas a maioria   perde esta propriedade ap&oacute;s o endurecimento. Uma vez que   a atividade antimicrobiana dos principais cimentos endod&ocirc;nticos   n&atilde;o &eacute; pronunciada e &eacute; ef&ecirc;mera <sup>30</sup>, &eacute; altamente improv&aacute;vel   que colabore na elimina&ccedil;&atilde;o de micro-organismos que sobreviveram aos efeitos do preparo qu&iacute;mico-mec&acirc;nico.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Na realidade, canais desinfetados devem ser obturados   para eliminar o espa&ccedil;o vazio que teria o potencial de ser infectado   ou reinfectado. Al&eacute;m disso, atrav&eacute;s de um selamento   tridimensional abrangendo os aspectos apical, lateral e coron&aacute;rio   do sistema de canais radiculares, a obtura&ccedil;&atilde;o pode   confinar micro-organismos residuais ao interior do canal,   impedindo seu egresso aos tecidos perirradiculares.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  A efic&aacute;cia de tal sepultamento microbiano vai depender   do n&uacute;mero de micro-organismos remanescentes no canal.   Em geral, isto pode ser aquilatado pelos resultados da cultura.   Casos com cultura negativa apresentam n&uacute;meros muito   baixos e n&atilde;o detect&aacute;veis de bact&eacute;rias cultiv&aacute;veis no canal, as   quais podem talvez ser sepultadas de forma eficaz pela obtura&ccedil;&atilde;o.   Casos de cultura positiva apresentam maior &iacute;ndice de   fracasso <sup>42,47</sup>, indicando que o sepultamento n&atilde;o funciona   bem em casos com maior n&uacute;mero de bact&eacute;rias residuais.   Isto refor&ccedil;a a necessidade de usar protocolos que previsivelmente   forne&ccedil;am culturas negativas antes da obtura&ccedil;&atilde;o.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  O selamento tridimensional do sistema de canais radiculares   tamb&eacute;m previne a recontamina&ccedil;&atilde;o do canal por   micro-organismos da saliva al&eacute;m de impedir a infiltra&ccedil;&atilde;o de   fluidos teciduais para o interior do canal, negando substrato   para micro-organismos sobreviventes.</font></p>     ]]></body>

Al&eacute;m de eliminar   micro-organismos presentes na luz do canal principal, a terapia antimicrobiana deve abranger uma estrat&eacute;gia que   possibilite a elimina&ccedil;&atilde;o de micro-organismos alojados em &aacute;reas mais distantes do canal principal, incluindo t&uacute;bulos dentin&aacute;rios, istmos, ramifica&ccedil;&otilde;es apicais, canais laterais e outras irregularidades <sup>23</sup>.</font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  O objetivo primordial do tratamento de dentes infectados &eacute; reduzir o n&uacute;mero de micro-organismos o m&aacute;ximo poss&iacute;vel, em   n&iacute;veis que sejam compat&iacute;veis com a repara&ccedil;&atilde;o perirradicular e manter estes n&iacute;veis baixos com a obtura&ccedil;&atilde;o. O protocolo para   tratar rotineiramente canais infectados &eacute; baseado tanto em evid&ecirc;ncias cient&iacute;ficas quanto na experi&ecirc;ncia cl&iacute;nica (<a href="#tab01">Tabela I</a>). Um excelente &iacute;ndice de sucesso (95% de repara&ccedil;&atilde;o) tem sido relatado para este protocolo <sup>40</sup>, mesmo quando usado por m&atilde;os inexperientes de alunos de gradua&ccedil;&atilde;o (<a href="#fig03">Figura 3</a>). Segue o protocolo:</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  1. O dente a ser tratado deve estar limpo de placa bacteriana e de c&aacute;lculo;    <br>   2. O preparo da cavidade de acesso pode ser iniciado sem isolamento absoluto. Isto facilita o procedimento e diminui riscos   de acidentes, mormente em dentes com inclina&ccedil;&atilde;o anormal. Todavia, ap&oacute;s a trepana&ccedil;&atilde;o do teto da c&acirc;mara pulpar e da amplia&ccedil;&atilde;o   da &aacute;rea de exposi&ccedil;&atilde;o, o isolamento deve ser aplicado antes da conclus&atilde;o das manobras de acesso;    <br>   3. Ap&oacute;s a aplica&ccedil;&atilde;o do isolamento absoluto, o campo operat&oacute;rio, incluindo dente, grampo e len&ccedil;ol de borracha, deve ser   inicialmente limpo com solu&ccedil;&atilde;o de per&oacute;xido de hidrog&ecirc;nio a 3% (&aacute;gua oxigenada 10 volumes) e ent&atilde;o descontaminado com   solu&ccedil;&atilde;o de &aacute;lcool iodado a 2%, clorexidina a 2% ou NaOCl a 2,5%;    <br>   4.Ap&oacute;s a conclus&atilde;o das manobras de acesso coron&aacute;rio, a c&acirc;mara pulpar deve ser copiosamente irrigada com solu&ccedil;&atilde;o de   NaOCl a 2,5%;    <br>   5. O preparo qu&iacute;mico-mec&acirc;nico deve ser realizado empregando-se uma t&eacute;cnica progressiva no sentido coroa-&aacute;pice, com   instrumentos manuais e/ou acionados a motor de n&iacute;quel-tit&acirc;nio associados &agrave; irriga&ccedil;&atilde;o copiosa e frequente com NaOCl a   2,5% ap&oacute;s cada uso de instrumento (no m&iacute;nimo 1 a 2 ml de solu&ccedil;&atilde;o irrigadora a cada troca de instrumento). O canal deve   ser ampliado na medida de 1 mm aqu&eacute;m do &aacute;pice radiogr&aacute;fico ou 1 mm aqu&eacute;m do forame, detectado por um localizador   apical eletr&ocirc;nico. Preparos amplos potencializam a desinfec&ccedil;&atilde;o, mas um meio termo tem que ser atingido para evitar o enfraquecimento   demasiado da estrutura dent&aacute;ria, que poderia predispor &agrave; fratura quando o dente se submeter aos esfor&ccedil;os   mastigat&oacute;rios. O segmento apical do canal que se estende do comprimento de trabalho at&eacute; o forame apical deve ser idealmente   limpo e mantido livre de detritos atrav&eacute;s do emprego das limas de pat&ecirc;ncia de pequeno calibre;    <br>   6. Remo&ccedil;&atilde;o da smear layer, pois a mesma pode conter bact&eacute;rias, pode impedir ou retardar a a&ccedil;&atilde;o em profundidade da medica&ccedil;&atilde;o   intracanal e interferir no selamento promovido pela futura obtura&ccedil;&atilde;o;    <br>   7. Em seguida, o canal deve ser inundado com NaOCl a 2,5%. Esta solu&ccedil;&atilde;o ent&atilde;o ser&aacute; ativada por 1 minuto usando-se uma   lima acoplada a um aparelho de ultrassom e que penetra livremente no canal at&eacute; o comprimento de trabalho. Uma lima   calibre 15 ou 20 pode ser usada em um canal preparado no comprimento de trabalho at&eacute; uma lima 35 a 45;    <br>   8. O canal deve ser medicado com a pasta HPG. Opcionalmente, pode-se utilizar a pasta de hidr&oacute;xido de c&aacute;lcio em clorexidina   de 0,12 a 2% (HCx). A aplica&ccedil;&atilde;o da pasta com espirais de lentulo acoplada a um micromotor de baixa-rota&ccedil;&atilde;o   confere resultados melhores de preenchimento do canal;9. Radiografa-se o dente para a verifica&ccedil;&atilde;o do preenchimento adequado do canal com a pasta HPG ou HCx. Limpa-se ent&atilde;o   a c&acirc;mara pulpar e ent&atilde;o aplica-se o selamento coron&aacute;rio com um cimento tempor&aacute;rio;    <br>   10. Na segunda sess&atilde;o, no m&iacute;nimo 5 a 7 dias depois, remove-se a pasta utilizando a lima de mem&oacute;ria associada &agrave; irriga&ccedil;&atilde;o   copiosa com NaOCl a 2,5 % e procede-se a obtura&ccedil;&atilde;o do canal. </font></p>     ]]></body>
