A revisão foi realizada através de busca de artigos originais sobre o referido assunto em periódicos listados na fonte Pubmed, desde 1990 até 2010. Foram selecionados artigos relacionados principalmente a: planejamento do caso, seleção de materiais, tipos e técnicas de preparo, tratamento das superfícies dente/restauração, cimentação e longevidade do procedimento. A fase mais crítica na técnica de facetas é a cimentação. A incorreta indicação do caso para o tratamento e o tipo de preparo utilizado também são apontados como principais alvos de falhas. A técnica de facetas laminadas é uma técnica sensível, porém, se forem respeitados os passos de sua confecção, apresenta alto índice de sucesso.]]></p></abstract>

However, the literature   shows that when footsteps are followed, it has a high success rate.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B>Keywords: </B>dental veneers; ceramics; dental restoration   failure.</font> </p> <hr noshade size="1">     <p>&nbsp;</p>     <p>&nbsp;</p>     <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><B> Introdu&ccedil;&atilde;o</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Os dentes anteriores t&ecirc;m import&acirc;ncia fundamental na est&eacute;tica da face   e, por isso, s&atilde;o extremamente valorizados pelos pacientes que desejam   clarear, aumentar, melhorar a anatomia ou posi&ccedil;&atilde;o dos mesmos,   na busca de um sorriso mais natural e harm&ocirc;nico <sup>18</sup>. A tend&ecirc;ncia atual &eacute; que   as modifica&ccedil;&otilde;es no sorriso sejam realizadas de maneira a haver um m&iacute;nimo   desgaste de estrutura dental sadia <sup>33,40</sup>. As facetas laminadas conseguiram   combinar o requisito conservador de seus preparos com suas qualidades de   resist&ecirc;ncia <sup>16</sup>, biocompatibilidade <sup>33</sup> e, principalmente, est&eacute;tica inigual&aacute;vel   <sup>16</sup>. No entanto, toda essa excel&ecirc;ncia est&eacute;tica s&oacute; poder&aacute; ser alcan&ccedil;ada se   houver conhecimento dos princ&iacute;pios b&aacute;sicos da est&eacute;tica dental, assim como o ex&iacute;mio dom&iacute;nio da t&eacute;cnica.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  O tratamento com facetas laminadas apresenta sensibilidade de t&eacute;cnica   desde a sele&ccedil;&atilde;o do caso at&eacute; o acabamento e polimento dessas restaura&ccedil;&otilde;es   <sup>5,14,28</sup>. A cimenta&ccedil;&atilde;o adesiva, utilizada nesse tipo de restaura&ccedil;&atilde;o, &eacute; a fase   mais cr&iacute;tica, apresentando detalhes que devem ser rigorosamente seguidos   para que se alcance uma ades&atilde;o adequada. Outros fatores tamb&eacute;m determinantes   do comprometimento do resultado final s&atilde;o: a escolha indevida do   tipo de preparo dental para as facetas, a falta de dom&iacute;nio da t&eacute;cnica durante   a confec&ccedil;&atilde;o do preparo e, ainda, a indica&ccedil;&atilde;o equivocada para esse tipo de   tratamento <sup>6,8,29</sup>.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  As falhas nos procedimentos odontol&oacute;gicos, por mais preparado e competente   que seja o cirurgi&atilde;o-dentista, podem acontecer. Assim, se os profissionais   est&atilde;o dispostos a aprender a partir dos insucessos encontrados,   ent&atilde;o os erros podem trazer benef&iacute;cios no sentido de ajudar no aprimoramento   das t&eacute;cnicas.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  O presente estudo teve como objetivo reunir e selecionar artigos dos &uacute;ltimos   vinte anos da literatura, a fim de realizar uma revis&atilde;o sobre falhas em   restaura&ccedil;&otilde;es de facetas laminadas. A partir deste trabalho, foi poss&iacute;vel orientar   os profissionais para as etapas mais cr&iacute;ticas durante a realiza&ccedil;&atilde;o da t&eacute;cnica,   com o objetivo de se evitar erros que possam comprometer o sucesso do   tratamento e a longevidade da restaura&ccedil;&atilde;o. </font></p>     <p>&nbsp;</p>     ]]></body>
<body><![CDATA[<p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><B> Material e M&eacute;todo</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Para realiza&ccedil;&atilde;o do estudo foram selecionados artigos originais sobre o referido   assunto em peri&oacute;dicos listados na fonte Pubmed, desde 1990 at&eacute; 2010.   Dentre estes, constam trabalhos longitudinais, de revis&atilde;o e pesquisas in vitro.   Para facilitar o entendimento, o estudo foi dividido e comentado em etapas   espec&iacute;ficas, onde, segundo a literatura, observam-se as principais falhas da   t&eacute;cnica de facetas laminadas: planejamento do caso, sele&ccedil;&atilde;o de materiais, tipos   e t&eacute;cnicas de preparo, tratamento das superf&iacute;cies dente/restaura&ccedil;&atilde;o, cimenta&ccedil;&atilde;o e longevidade do procedimento.</font></p>     <p>&nbsp;</p>     <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><B> Revis&atilde;o da Literatura e Discuss&atilde;o</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B>&bull; Planejamento do Caso</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Uma etapa fundamental para a confec&ccedil;&atilde;o de qualquer tipo de trabalho   restaurador &eacute; o planejamento <sup>21,29,30</sup>. Este possibilita a previsibilidade do tratamento, permitindo que poss&iacute;veis falhas sejam minimizadas.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  A primeira possibilidade de falha desta t&eacute;cnica &eacute; a sele&ccedil;&atilde;o   equivocada do caso, pois o respeito &agrave;s suas indica&ccedil;&otilde;es&eacute;  condi&ccedil;&atilde;o indispens&aacute;vel para o sucesso do tratamento. Assim,   as facetas est&atilde;o indicadas para restaura&ccedil;&otilde;es de elementos   dentais com: altera&ccedil;&atilde;o de cor, forma, tamanho, posi&ccedil;&atilde;o,   em faces vestibulares com les&otilde;es cariosas ou restaura&ccedil;&otilde;es   deficientes e em fechamento de diastemas <sup>11,13,28</sup>. Outra   indica&ccedil;&atilde;o seria a altera&ccedil;&atilde;o e corre&ccedil;&atilde;o de rela&ccedil;&otilde;es oclusais   como mudan&ccedil;a da guia e dimens&atilde;o vertical <sup>20</sup>.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  As falhas podem ocorrem quando n&atilde;o se respeita as limita&ccedil;&otilde;es   do tratamento com facetas, como, por exemplo,   pacientes com h&aacute;bitos parafuncionais, dentes com estrutura   coron&aacute;ria reduzida <sup>15</sup>, dentes muito vestibularizados e que   apresentam grande apinhamento ou girovers&atilde;o <sup>13</sup>. Al&eacute;m   disso, o aumento de falhas pode ser observado quando facetas   s&atilde;o colocadas sobre amplas restaura&ccedil;&otilde;es j&aacute; existentes   <sup>15</sup> ou tamb&eacute;m em casos de diastema exagerado. Estes podem   causar problemas est&eacute;ticos relacionados &agrave; desarmonia   do sorriso pela despropor&ccedil;&atilde;o entre os elementos <sup>21</sup>. Outros   fatores relacionados que tamb&eacute;m podem limitar a utiliza&ccedil;&atilde;o   das facetas laminadas s&atilde;o inflama&ccedil;&otilde;es periodontais   e inser&ccedil;&atilde;o baixa de freio labial <sup>21</sup>. Nesse caso, um planejamento   com aux&iacute;lio da periodontia e ortodontia certamente   resultar&aacute; em sucesso do tratamento.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Outro ponto do planejamento que tamb&eacute;m influenciar&aacute;  no sucesso do tratamento s&atilde;o as expectativas e desejos do   paciente em rela&ccedil;&atilde;o ao tratamento dental <sup>5,21</sup>. Nesta etapa,   o profissional deve lan&ccedil;ar m&atilde;o de enceramentos, mock-ups, imagens computadorizadas e provis&oacute;rios de excelente   qualidade para que o paciente observe e opine durante   o planejamento, a fim de que o cirurgi&atilde;o-dentista consiga   chegar ao resultado final esperado <sup>21</sup>. </font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B>&bull; Sele&ccedil;&atilde;o dos Materiais</B></font></p>       ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">As facetas podem ser realizadas com diversos materiais   <sup>17,25,34</sup>. Atualmente, utiliza-se resina composta direta   ou indiretamente e porcelana. Assim, comparando-se os   materiais, observa-se que, em curto prazo, eles apresentam   resultados semelhantes. No entanto, a m&eacute;dio e longo prazo,   a baixa resist&ecirc;ncia das facetas de resina &eacute; evidenciada, resultando   em trincas e fraturas, al&eacute;m de manchamentos devido  &agrave; porosidade do material <sup>34</sup>. Outras vantagens das facetas   de porcelana s&atilde;o a estabilidade de cor, resist&ecirc;ncia ao desgaste e longevidade est&eacute;tica <sup>16</sup>.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Os estudos longitudinais comparativos com 5 a 10 anos   de dura&ccedil;&atilde;o mostram todos os seus resultados favor&aacute;veis   apenas a facetas de porcelana, com uma m&eacute;dia de sucesso   de 95 a 99% <sup>5,14,16,19,24,33</sup>. Contudo, existem v&aacute;rios   tipos de cer&acirc;micas sendo importante o conhecimento de   suas caracter&iacute;sticas para uma menor chance de erro durante   a escolha do material em cada situa&ccedil;&atilde;o cl&iacute;nica. A literatura   apresenta uma divis&atilde;o para sele&ccedil;&atilde;o do tipo de material baseada   na necessidade est&eacute;tica e funcional do paciente <sup>18</sup>:   tipo 1-A: pacientes cujas facetas n&atilde;o ser&atilde;o expostas a carga   funcional e dentes sem altera&ccedil;&atilde;o de cor; tipo 1-B: pacientes   cujas facetas n&atilde;o ser&atilde;o expostas a carga funcional e dentes   com altera&ccedil;&atilde;o de cor; tipo 2: pacientes cujas facetas ser&atilde;o   expostas a carga funcional. Os pacientes do tipo 1-A s&atilde;o   candidatos a facetas de cer&acirc;mica feldsp&aacute;tica convencional,   devido &agrave;s excelentes caracter&iacute;sticas est&eacute;ticas. A exce&ccedil;&atilde;o &eacute;  quando o paciente apresenta diastemas, de moderados a   grandes (mais de 2 mm), pois a porcelana ir&aacute; se estender   al&eacute;m da &aacute;rea de ades&atilde;o. Nesses casos &eacute; recomendado o uso   de cer&acirc;micas feldsp&aacute;ticas de alta resist&ecirc;ncia (refor&ccedil;ada com   cristais de leucita e/ou refor&ccedil;ada com &oacute;xido de l&iacute;tio), j&aacute; que   as boas qualidades est&eacute;ticas se somam &agrave; adequada resist&ecirc;ncia  &agrave; fratura <sup>18</sup>. Em pacientes do tipo 1-B devem-se utilizar   porcelanas e cimentos que possuam graus de opacidade   (cer&acirc;micas de &oacute;xido de alum&iacute;nio e/ou de &oacute;xido de zirc&ocirc;nia),   para que possam mascarar a altera&ccedil;&atilde;o de cor dos dentes   <sup>18</sup>. Contudo, isso implica no comprometimento de efeitos  &oacute;pticos em termos de translucidez e reflex&atilde;o e, consequentemente,   da est&eacute;tica da restaura&ccedil;&atilde;o. A aplica&ccedil;&atilde;o cl&iacute;nica do   Procera e do Empress II em laminados, talvez, seja ainda   um pouco limitada em dentes com severa altera&ccedil;&atilde;o de cor   <sup>12</sup>. Alguns autores consideram o Procera como a melhor   op&ccedil;&atilde;o, pois o casquete de alumina concede uma maior durabilidade   e previsibilidade na escolha da cor <sup>23</sup>. Outros   fatores a serem considerados nesses casos s&atilde;o a linha de t&eacute;rmino   e o preparo dental. Especificamente nesses elementos   com altera&ccedil;&atilde;o de cor, a linha do t&eacute;rmino deve ser realizada   levemente subgengival e o preparo dental dever&aacute; ser mais   agressivo (0.8-1 mm). Quando comparadas a espessura de   laminados cer&acirc;micos com o grau de opacidade dos mesmos,   observamos que a espessura da faceta de porcelana &eacute; o fator   prim&aacute;rio que afeta a transmiss&atilde;o de luz atrav&eacute;s das facetas   <sup>32</sup>. Em pacientes do tipo 2, a exposi&ccedil;&atilde;o de facetas &agrave; carga   funcional requer materiais de alta resist&ecirc;ncia &agrave; fratura. Sendo   assim, a indica&ccedil;&atilde;o &eacute; de cer&acirc;micas feldsp&aacute;ticas ou de alumina   de alta resist&ecirc;ncia devido as suas propriedades est&eacute;ticas e previsibilidade   em reabilita&ccedil;&atilde;o oral da guia anterior <sup>18</sup>.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B>&bull; Tipos de Preparo</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">  Existe uma gama de tipos de preparo para facetas laminadas   descritos na literatura <sup>6,13,22,36</sup>. A profundidade   varia de 0 a 1,0 mm, sendo o desgaste m&eacute;dio de 0,4 mm   a 0,7 mm <sup>13</sup>. Assim, comparando-se os diferentes tipos  &eacute; constatado que os preparos do tipo: janela (sem redu&ccedil;&atilde;o   incisal); preparo com cobertura incisal de 2 mm; preparo   sem chanfro na palatina (justaposto) apresentaram os melhores   resultados em rela&ccedil;&atilde;o &agrave; resist&ecirc;ncia &agrave; carga e &agrave; fratura   dos laminados cer&acirc;micos <sup>6</sup>. Isto ocorre porque restaura&ccedil;&otilde;es   com chanfro longo estendendo-se para a concavidade   palatal permitem que finas extens&otilde;es de cer&acirc;mica fiquem   sujeitas &agrave;s maiores for&ccedil;as de tens&atilde;o causadas pela geometria   da regi&atilde;o palatal. Tamb&eacute;m, minichanfros ou margens   justapostas s&atilde;o recomendados, especialmente, em fratura de coroas ou desgaste incisal severo <sup>27,28</sup>. M&aacute;ximas for&ccedil;as   de tens&atilde;o e compress&atilde;o s&atilde;o registradas na regi&atilde;o cervical   das restaura&ccedil;&otilde;es de facetas. Assim, essa regi&atilde;o do preparo   pode apresentar tr&ecirc;s formatos: chanfro, ombro ou l&acirc;mina   de faca. Comparando-se os tr&ecirc;s &eacute; constatado que as facetas   de porcelana com cobertura incisal e com t&eacute;rmino cervical   do tipo l&acirc;mina de faca podem suportar melhor as for&ccedil;as   oclusais sem fraturar <sup>36</sup>. O posicionamento das margens   do preparo tamb&eacute;m &eacute; importante alvo de falhas das facetas   laminadas de porcelana <sup>21</sup>. &Eacute; fundamental a observa&ccedil;&atilde;o   do preparo por todos os &acirc;ngulos do sorriso do paciente para   que a linha de cimenta&ccedil;&atilde;o n&atilde;o fique aparente <sup>21</sup>. </font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B>&bull; T&eacute;cnicas de Preparo Dental</B></font></p>       <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">O preparo incorreto pode ser apontado como o componente   principal para fraturas causadas por falhas coesivas   e adesivas, pois o desgaste insuficiente pode n&atilde;o criar espa&ccedil;o   adequado para a faceta de porcelana <sup>21</sup>, enquanto o   desgaste excessivo pode remover &aacute;reas de esmalte, prejudicando   a ades&atilde;o <sup>9</sup>. Al&eacute;m disso, a realiza&ccedil;&atilde;o de um desgaste   homog&ecirc;neo &eacute; fundamental para que a cer&acirc;mica apresente a   mesma espessura em toda sua extens&atilde;o, tornando-se mais   resistente. As tr&ecirc;s principais t&eacute;cnicas descritas para realiza&ccedil;&atilde;o   do preparo de facetas s&atilde;o: m&atilde;o livre, guia de desgaste   com sulcos de orienta&ccedil;&atilde;o e covinha. O m&eacute;todo dos sulcos de   orienta&ccedil;&atilde;o consiste na marca&ccedil;&atilde;o com brocas tronco-c&ocirc;nicas   de extremo arredondado de sulcos com a profundidade desejada.   Uma varia&ccedil;&atilde;o desse m&eacute;todo &eacute; a confec&ccedil;&atilde;o de apenas   um sulco central. O terceiro m&eacute;todo se utiliza de brocas esf&eacute;ricas   de pequeno di&acirc;metro para marcar em v&aacute;rios pontos   do preparo o limite de desgaste. Comparando-se as t&eacute;cnicas,   o uso de brocas esf&eacute;ricas de pequeno di&acirc;metro (covinha) e a   confec&ccedil;&atilde;o de apenas um sulco central s&atilde;o as que se aproximam da profundidade ideal <sup>8</sup>.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B>&bull; Tratamento de Superf&iacute;cie da Cer&acirc;mica</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> O tratamento de superf&iacute;cie dos laminados cer&acirc;micos visa   uma maior ades&atilde;o entre este e o cimento resinoso. Uma das   formas de preparo da superf&iacute;cie interna da faceta &eacute; o jateamento   com &oacute;xido de alum&iacute;nio. Esse tipo de tratamento cria   rugosidades na superf&iacute;cie da cer&acirc;mica, aumentando a &aacute;rea   de contato com o cimento resinoso <sup>13</sup>. Outro tipo de tratamento  &eacute; o condicionamento &aacute;cido da superf&iacute;cie interna de   facetas com o &aacute;cido hidrofluor&iacute;drico, cuja fun&ccedil;&atilde;o &eacute; de criar   microporosidades na superf&iacute;cie, promover uma limpeza da   pe&ccedil;a, melhorarando a ades&atilde;o <sup>13</sup>. O condicionamento tamb&eacute;m   promove uma redu&ccedil;&atilde;o significativa da for&ccedil;a flexural em   fun&ccedil;&atilde;o do tempo de aplica&ccedil;&atilde;o e da concentra&ccedil;&atilde;o do &aacute;cido,   al&eacute;m do aumento na dureza de superf&iacute;cie associado &agrave; concentra&ccedil;&atilde;o   do mesmo <sup>2</sup>. Segundo a literatura, a longevidade   das facetas laminadas de cer&acirc;mica pode ser otimizada, se   forem aumentados o per&iacute;odo de condicionamento e a concentra&ccedil;&atilde;o   do &aacute;cido <sup>2</sup>. A concentra&ccedil;&atilde;o do &aacute;cido atualmente   preconizada &eacute; de 2 a 10% e o tempo estimado de condicionamento  &eacute; de 20 segundos ou 2 minutos, dependendo do tipo   de cer&acirc;mica utilizada <sup>13</sup>. &Eacute; v&aacute;lido ressaltar que determinados   tipos de cer&acirc;mica pobres em s&iacute;lica, como o sistema   Procera, n&atilde;o s&atilde;o pass&iacute;veis de condicionamento, logo outras   formas de tratamento de superf&iacute;cie devem ser empregadas.   As facetas de porcelana tratadas com abrasivos de &oacute;xido   de alum&iacute;nio apresentam maior resist&ecirc;ncia &agrave; fratura quando   comparadas a facetas tratadas apenas com &aacute;cido hidrofluor&iacute;drico  <sup>1</sup>. Para se evitar falhas adesivas o ideal &eacute; que   se realize o jateamento abrasivo com &oacute;xido de alum&iacute;nio, e,   posteriormente, o condicionamento com o &aacute;cido fluor&iacute;drico,   seguido de silaniza&ccedil;&atilde;o da superf&iacute;cie de porcelana <sup>21</sup>. </font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B>&bull; Tratamento de Superf&iacute;cie do Dente</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Quando se trata de condicionamento da superf&iacute;cie dental   para cimenta&ccedil;&atilde;o de facetas, &eacute; indispens&aacute;vel o isolamento do   campo operat&oacute;rio, na medida em que controla a umidade e   impede qualquer tipo de contamina&ccedil;&atilde;o da superf&iacute;cie dental.   A estrutura dental deve ser condicionada com &aacute;cido fosf&oacute;rico   37% por 15 segundos, seguida de lavagem e secagem   da superf&iacute;cie para que o sistema adesivo possa ser aplicado   <sup>13</sup>. A exposi&ccedil;&atilde;o &agrave; umidade e aos contaminantes da cavidade   oral pode causar falhas adesivas, resultando em um alto   percentual de descolamento das facetas laminadas de porcelana   na interface dente-resina. Tamb&eacute;m, o preparo, anteriormente   discutido, sendo totalmente realizado em esmalte  &eacute; um fator que ajuda no sucesso da ades&atilde;o <sup>21</sup>. Segundo   SEYMOR <sup>36</sup>, a interface cimento resinoso &ndash; dente &eacute; o primeiro   lugar de entrada do fluido oral. Assim, a falha adesiva   parece ocorrer quando 80% ou mais do substrato &eacute; de dentina,   sendo altamente improv&aacute;vel quando um m&iacute;nimo de 0,5 mm de esmalte &eacute; preservado <sup>35</sup>.</font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B>&bull; Agente Cimentante</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> A fase de cimenta&ccedil;&atilde;o &eacute; uma das mais cr&iacute;ticas na t&eacute;cnica   de facetas laminadas de porcelana, j&aacute; que esta envolve   muitos passos. Primeiramente, FRADEANI et al. <sup>19</sup> j&aacute;  apontam que para se obter alta taxa de sucesso e longevidade,   a cimenta&ccedil;&atilde;o tem que ser adesiva. Em rela&ccedil;&atilde;o &agrave; escolha   do cimento resinoso, &eacute; dif&iacute;cil eleger um ideal, pois al&eacute;m   de haver v&aacute;rios tipos e marcas, muitos produtos entram e   saem do mercado com relativa rapidez. Alguns autores <sup>7,27,28</sup> apontam que as principais falhas n&atilde;o dependem da   marca do agente cimentante escolhido, mas dos passos da   cimenta&ccedil;&atilde;o. BARCELEIRO <sup>4</sup> ainda sugere como agente cimentante   a resina flow fotopolimeriz&aacute;vel, j&aacute; que a m&eacute;dia de   resist&ecirc;ncia ao cisalhamento desta em rela&ccedil;&atilde;o a um cimento   resinoso, n&atilde;o &eacute; considerada estatisticamente diferente <sup>4</sup>.   Outro alvo de falhas na fase de escolha do agente cimentante  &eacute; a cor. A escolha equivocada e a mudan&ccedil;a de colora&ccedil;&atilde;o   com o tempo tamb&eacute;m podem ser apontadas como raz&otilde;es de   falha das facetas laminadas <sup>26,28</sup>. A melhor maneira de se   evitar estes erros &eacute; atrav&eacute;s de uma boa comunica&ccedil;&atilde;o com o   t&eacute;cnico ceramista, a utiliza&ccedil;&atilde;o de fotografias <sup>21</sup> e a sele&ccedil;&atilde;o   de um agente cimentante com cor adequada. Os laminados cer&acirc;micos possuem pequena espessura, por isso, a cor resultante desse tipo de restaura&ccedil;&atilde;o &eacute; proveniente da combina&ccedil;&atilde;o de tr&ecirc;s substratos: remanescente dental, cer&acirc;mica e cimento <sup>10,11</sup>. Para facilitar a escolha de cor, os fabricantes oferecem bisnagas de cimento para testes de cor, os chamados cimentos "try-in". Al&eacute;m disso, &eacute; interessante citar que a cimenta&ccedil;&atilde;o das facetas laminadas deve ser realizada preferencialmente com cimentos fotoativados, j&aacute; que sua amina alif&aacute;tica &eacute; mais est&aacute;vel que a arom&aacute;tica presente nos agentes de cimenta&ccedil;&atilde;o qu&iacute;mica. Dessa forma, estes apresentam maior estabilidade de cor e permitem um melhor controle do tempo de trabalho <sup>13</sup> (<a href="#fig01">Figura 1</a>). </font></p>     <p>&nbsp;</p>     <p><a name="fig01"></a></p>     <p>&nbsp; </p>     <p align="center"><img src="/img/revistas/rbo/v69n1/a11fig01.jpg">     <p>&nbsp;</p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B>&bull; Cimenta&ccedil;&atilde;o</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif">Para a realiza&ccedil;&atilde;o de uma t&eacute;cnica de cimenta&ccedil;&atilde;o ideal, o   cimento deve ser aplicado de maneira homog&ecirc;nea em toda   a extens&atilde;o do laminado cer&acirc;mico. Isso evita que haja falta   de cimento em alguma regi&atilde;o do preparo, o que poderia   resultar em altera&ccedil;&otilde;es de cor e, principalmente, em falhas   adesivas <sup>13</sup>. MAGNE &amp; DOUGLAS <sup>27,31</sup> afirmam ainda   que a espessura do cimento tem grande influ&ecirc;ncia na distribui&ccedil;&atilde;o   das tens&otilde;es nas restaura&ccedil;&otilde;es. Assim, facetas demasiadamente   finas combinadas com cobertura interna pobre   resultam em aumento de tens&atilde;o tanto na superf&iacute;cie como na   interface da restaura&ccedil;&atilde;o. O m&eacute;todo tradicional de cimenta&ccedil;&atilde;o   em que o adesivo dentin&aacute;rio &eacute; aplicado e polimerizado   com o laminado em posi&ccedil;&atilde;o &eacute; associado a falhas de ades&atilde;o   entre a camada h&iacute;brida e a resina superposta. Um m&eacute;todo   alternativo no qual o adesivo dentin&aacute;rio &eacute; aplicado &agrave; dentina   e polimerizado antes de cimenta&ccedil;&atilde;o tem se mostrado mais   efetivo para o restabelecimento do comportamento biomim&eacute;tico   do elemento dental <sup>28</sup>. Por&eacute;m, a camada de adesivo   aplicada deve ser bastante fina ou a adapta&ccedil;&atilde;o da faceta poder&aacute;  ser prejudicada <sup>3</sup>. Outro potencial fator causador de   falhas na cimenta&ccedil;&atilde;o &eacute; a contra&ccedil;&atilde;o do cimento resinoso, que   pode gerar for&ccedil;as compressivas na cer&acirc;mica e na superf&iacute;cie   do dente, culminando em fratura do laminado. Quando o   aumento de temperatura ocorre, combinado com a contra&ccedil;&atilde;o,   a for&ccedil;a compressiva diminuiu ao menor par&acirc;metro, aumentando   a for&ccedil;a de tens&atilde;o, podendo ocorrer a quebra da   restaura&ccedil;&atilde;o. Devido &agrave; fragilidade da cer&acirc;mica, essas for&ccedil;as   de tens&atilde;o s&atilde;o classificadas como as mais prejudiciais, sendo   influenciadas, principalmente pela cobertura vestibular <sup>27,29</sup>. Assim, &eacute; fundamental que sejam mantidos os aspectos   de configura&ccedil;&atilde;o favor&aacute;vel dessa restaura&ccedil;&atilde;o, j&aacute; descritos   anteriormente, para que este tipo de falha n&atilde;o ocorra. As   falhas adesivas e coesivas ou alguma combina&ccedil;&atilde;o dessas falhas   podem causar fraturas <sup>21</sup>. Quando a faceta laminada   de porcelana obtiver falha de ades&atilde;o, esta pode se quebrar   como resultado, principalmente de est&iacute;mulos externos.   Nesses casos, normalmente uma larga por&ccedil;&atilde;o da restaura&ccedil;&atilde;o  &eacute; quebrada. Fraturas coesivas podem ser causadas por   trauma a faceta. Se a ades&atilde;o for completada com sucesso,   ent&atilde;o, apenas uma pequena parte da restaura&ccedil;&atilde;o ir&aacute; sofrer   uma lasca <sup>21</sup>. A fratura que &eacute; relacionada para ambas as   falhas adesivas e coesivas est&aacute; geralmente ligada a origens iatrog&ecirc;nicas <sup>21</sup>.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B>&bull; Acabamento, Polimento e Selamento</B></font></p>     ]]></body>
<body><![CDATA[<p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Todos os ajustes nas facetas de porcelana devem ser corrigidos   antes da cimenta&ccedil;&atilde;o para que o laborat&oacute;rio possa   aplicar o glaze final na superf&iacute;cie externa da restaura&ccedil;&atilde;o   <sup>21</sup>. Corre&ccedil;&otilde;es realizadas ap&oacute;s esse procedimento resultam   em uma superf&iacute;cie rugosa e sem brilho, estando mais   propensa a manchamentos por colora&ccedil;&atilde;o extr&iacute;nseca <sup>21</sup>.&Eacute;  importante ressaltar, tamb&eacute;m, que quanto mais rugosa for   a superf&iacute;cie da porcelana, menor ser&aacute; a resist&ecirc;ncia flexural   da mesma. Sendo assim, &eacute; recomendado que as facetas de   cer&acirc;mica sejam cuidadosamente polidas <sup>17</sup>. Embora seja   raro o desenvolvimento de les&otilde;es de c&aacute;rie nas margens da   restaura&ccedil;&atilde;o, com exce&ccedil;&atilde;o dos pacientes com alta atividade   de les&otilde;es cariosas <sup>35</sup>, o selamento das margens finais da   restaura&ccedil;&atilde;o de porcelana reduz a infiltra&ccedil;&atilde;o em todas as interfaces <sup>39</sup>.</font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><B>&bull; Longevidade</B></font></p>     <p><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> A longevidade das facetas laminadas de porcelana vem   sendo questionada e investigada pelos pesquisadores desde   sua cria&ccedil;&atilde;o. A cada aperfei&ccedil;oamento da t&eacute;cnica s&atilde;o realizados   mais estudos visando observar o aumento da durabilidade   da restaura&ccedil;&atilde;o e melhora das suas propriedades de resist&ecirc;ncia   e est&eacute;tica. Os estudos encontrados na literatura que   acompanharam por mais tempo estas restaura&ccedil;&otilde;es duraram   de 10 a 15 anos e os resultados encontrados foram promissores   <sup>5,14,19,20,33</sup> (<a href="#graf01">Gr&aacute;fico 1</a>). Apesar da taxa de fraturas   ser considerada muito baixa pela maioria dos autores <sup>15,22,29.36,37</sup>, elas ocorrem principalmente por falha no   protocolo t&eacute;cnico. As principais causas de falhas relativas   s&atilde;o trincas, fraturas coesivas na cer&acirc;mica e falha na ades&atilde;o.   Os m&iacute;nimos erros durante a realiza&ccedil;&atilde;o das facetas podem   levar a posteriores fraturas. Assim, a longevidade das facetas   laminadas est&aacute; diretamente ligada &agrave; realiza&ccedil;&atilde;o de uma   t&eacute;cnica detalhada, com confec&ccedil;&atilde;o e cimenta&ccedil;&atilde;o adequadas,   al&eacute;m de cuidados do paciente e profissional para com a manuten&ccedil;&atilde;o da restaura&ccedil;&atilde;o <sup>5,14,27,33</sup> (Figuras <a href="#fig02">2</a> e <a href="#fig03">3</a>). </font></p>     <p>&nbsp;</p>     <p><a name="fig02"></a></p>     <p>&nbsp; </p>     <p align="center"><img src="/img/revistas/rbo/v69n1/a11fig02.jpg">     <p>&nbsp;</p>     <p><a name="fig03"></a></p>     <p>&nbsp; </p>     ]]></body>
