MÉTODOS: Quarenta incisivos inferiores extraídos, com um único canal, tiveram seus condutos radiculares preparados biomecanicamente, a 2,0 mm aquém do ápice radicular, com o sistema ProTaper®; finalizado com o instrumento F3. A irrigação foi executada com 1,0 mL de NaOCl a 2,5% a cada troca de calibre de instrumento e, ao final, com 5,0 mL de soro fisiológico. Concluída a instrumentação, o canal cementário foi padronizado com uma lima K 25 até que sua guia de penetração aparecesse na abertura foraminal. A partir deste momento, os dentes foram subdivididos em dois grupos de vinte espécimes cada, em função do método de irrigação final utilizado: I - irrigação com EDTA a 17% por 3min e II - idêntico protocolo, porém após a aspiração do EDTA a 17%, novamente irrigado com o digluconato de clorexidina a 2%, também mantido intracanal, por 3min. Cada grupo foi novamente subdivido em dois subgrupos (dez dentes cada), conforme o cimento empregado (Sealer 26® ou Sealapex®), e obturado pela técnica do cone único, com o cone F1 (ProTaper®; Universal Obturação), de tal forma que este ultrapassasse 2,0 mm além do ápice radicular. Imediatamente após, os dentes foram imersos em Rhodamine B® por 72h, sob vácuo, e após o desgaste das raízes no sentido vestibulolingual, as infiltrações demarcadas foram medidas com o programa Image Tool®. RESULTADOS E CONCLUSÃO: As magnitudes das infiltrações assemelharam-se entre si (p &gt; 0,05), à exceção do próprio grupo do cimento Sealapex®; pois o uso final com a clorexidina reduziu a infiltração apical (p < 0,05).]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[OBJECTIVE: The objective of this paper was to verify if the final irrigation at 17% EDTA, separately or followed by chlorhexidine digluconate at 2% interferes on the apical marginal leakage in root canal overfilling, due to the use of two root canal filling materials (Sealer 26TM or SealapexTM). METHODS: Forty lower incisors extracted, with a single root canal, were biomechanically prepared, at 2.0mm beyond the radicular apex, with ProTaper SystemTM; finishing it with the F3 instrument. Irrigation was accomplished with 1.0 mL NaOCl at 2.5% at each change of equipment caliper and, in the end, with 5.0 mL normal saline solution. After achieving this procedure, the foraminal channels was standardized with a file K 25 until its DO appears in the foraminal opening. From this moment, the teeth were subdivided into two groups of 20 specimens each, because of the final irrigation method used: I - irrigation with EDTA at 17% for 3min and II - identical protocol, thus, after EDTA aspiration at17%, it was again irrigated with chlorhexidine digluconate at 2%, also kept intra-canal for 3min. Each group was again sub-divided into two sub-groups (ten teeth each), according to the cement used (Sealer 26TM or SealapexTM), and filled by using a single gutta-percha F1 (ProTaperTM; Universal Filling), in such a manner that it goes 2.0 mm beyond the radicular apex. Soon after that, the teeth were immersed in Rhodamine B TM for 72h, vacuum and after the roots have been bucolingually grooved, the leakages marked were measured with program Image ToolTM. RESULTS AND CONCLUSIONS: The leakage magnitude was similar among themselves (p &gt; 0.05), except for group Sealapex®; thus the final use with chlorhexidine reduced apical leakage (p < 0.05).]]></p></abstract>

Endodontics materials.</font></p> <hr size="1" noshade>     <p>&nbsp;</p>     <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Introdu&ccedil;&atilde;o</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">O cl&aacute;ssico    trabalho de Kuttler<sup>1</sup> (1955) cita que a anatomia do forame apical    &eacute; constitu&iacute;da pela maior abertura foraminal e menor di&acirc;metro    da constri&ccedil;&atilde;o, em m&eacute;dia, localizado a 0,5 mm do &aacute;pice    radicular, sendo este local o recomendado como o limite apical da obtura&ccedil;&atilde;o<sup>2</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">A anatomia apical    est&aacute; comprometida em casos de reabsor&ccedil;&atilde;o radicular, sendo    pertinente executar o "desbridamento foraminal" visando ao saneamento desta    regi&atilde;o. Ap&oacute;s o uso do instrumento apical foraminal, deve-se recuar,    em m&eacute;dia, 1 mm aqu&eacute;m da extens&atilde;o radicular a fim de definir    o degrau apical.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Com a dilata&ccedil;&atilde;o    dessa &aacute;rea h&aacute; o risco de ocorrer o extravasamento apical da obtura&ccedil;&atilde;o,    principalmente com a ultrapassagem do cone principal de guta percha al&eacute;m    do &aacute;pice radicular, o que reduziria o &iacute;ndice de sucesso dos tratamentos    endod&ocirc;nticos<sup>3</sup>. Idealmente, quando constatado este acidente    t&eacute;cnico, a remo&ccedil;&atilde;o da obtura&ccedil;&atilde;o deve ser    imediata. Por&eacute;m, ao existir risco de fratura do cone de guta percha na    tentativa de sua remo&ccedil;&atilde;o, &eacute; prefer&iacute;vel mant&ecirc;-lo    e apenas acompanhar cl&iacute;nica e radiograficamente.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Portanto, quando    &eacute; presum&iacute;vel a ocorr&ecirc;ncia da complica&ccedil;&atilde;o,    todo cuidado deve ser tomado para preveni-la, assim como todos os recursos dever&atilde;o    ser empregados para que, caso venha a ocorrer, tenha o melhor progn&oacute;stico    poss&iacute;vel. Nessas situa&ccedil;&otilde;es, como cautela, empregam-se cimentos    endod&ocirc;nticos que tenham boa toler&acirc;ncia biol&oacute;gica e adequada    capacidade de selamento apical, principalmente os que contenham c&aacute;lcio    em sua composi&ccedil;&atilde;o<sup>4,5</sup>; associados a um material biocompat&iacute;vel,    como os cones de guta percha ou de pol&iacute;mero sint&eacute;tico, tipo Resilon<sup>&reg;6</sup>.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Contudo, em condi&ccedil;&otilde;es    de sobreobtura&ccedil;&atilde;o haver&aacute; uma linha de cimento entre o cone    de guta percha e a parede da abertura foraminal. Possivelmente, quanto maiores    por&eacute;m a capacidade de vedamento do cimento obturador e o embricamento    com a dentina subjacente, menor ser&aacute; a infiltra&ccedil;&atilde;o apical,    com menos danos ao processo de reparo perirradicular.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Portanto, objetiva-se,    por meio do presente estudo, avaliar a interfer&ecirc;ncia do agente de irriga&ccedil;&atilde;o    final EDTA isoladamente ou procedida da clorexidina com os cimentos endod&ocirc;nticos    Sealer 26<sup>&reg;</sup> ou Sealapex<sup>&reg;</sup> em situa&ccedil;&otilde;es    de sobreobtura&ccedil;&otilde;es endod&ocirc;nticas.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Materiais e    m&eacute;todo</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Ap&oacute;s a devida    aprova&ccedil;&atilde;o do presente estudo pelo Comit&ecirc; de &Eacute;tica    da Funda&ccedil;&atilde;o Municipal de Educa&ccedil;&atilde;o e Cultura de Santa    F&eacute; do Sul - SP (proc. 0000033/09), as aberturas coron&aacute;rias de    quarenta incisivos inferiores humanos extra&iacute;dos foram realizadas com    aux&iacute;lio de uma ponta diamantada 1012 (KG Sorensen<sup>&reg;</sup>; S&atilde;o    Paulo - SP, Brasil) e complementadas com a 3080 (KG Sorensen<sup>&reg;</sup>;    S&atilde;o Paulo - SP, Brasil) em alta rota&ccedil;&atilde;o.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Inicialmente foi    realizada a explora&ccedil;&atilde;o inicial com limas K10 e K15 at&eacute;    que a sua guia de penetra&ccedil;&atilde;o aparecesse na abertura foraminal.    Esta medida foi determinada como o comprimento real do dente. Recuando-se 2    mm desta extens&atilde;o, foi estabelecido o limite apical de instrumenta&ccedil;&atilde;o.    Os canais radiculares foram instrumentados com o sistema ProTaper<sup>&reg;</sup>    (Dentsply Maillefer, Petr&oacute;polis - RJ, Brasil) acionados com o Endomate    TC<sup>&reg;</sup> (NSK, Nakanishi Inc., Jap&atilde;o), com torque de 1,5 N.cm    e em 250 rpm de velocidade.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">O instrumento S1    foi empregado at&eacute; o in&iacute;cio do ter&ccedil;o m&eacute;dio radicular.    Na sequ&ecirc;ncia, as embocaduras dos canais radiculares foram ampliadas com    a utiliza&ccedil;&atilde;o do instrumento Sx e, sucessivamente, com as brocas    de Gates Glidden 2 e 1.O instrumento S2 foi ent&atilde;o empregado at&eacute;    o in&iacute;cio do ter&ccedil;o apical radicular e, sequencialmente, os instrumentos    F<sub>1</sub>, F<sub>2</sub> e F<sub>3</sub> no comprimento real de instrumenta&ccedil;&atilde;o.    O preparo biomec&acirc;nico foi executado empregando-se 1 mL de NaOCl a 2,5%    a cada troca de instrumento e conclu&iacute;do com a irriga&ccedil;&atilde;o    final com 5,0 mL de soro fisiol&oacute;gico. Conclu&iacute;da a instrumenta&ccedil;&atilde;o    dos canais, padronizou-se o canal cement&aacute;rio das ra&iacute;zes com a    lima K25, em movimento oscilat&oacute;rio, at&eacute; que a sua guia de penetra&ccedil;&atilde;o    fosse vis&iacute;vel na abertura foraminal.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Os dentes foram    divididos em dois grupos, sendo vinte dentes irrigados com EDTA triss&oacute;dico    a 17% (Biodin&acirc;mica<sup>&reg;</sup>; Ibipor&atilde; - PR, Brasil), mantido    por 3min e neutralizado com 1,0 mL de soro fisiol&oacute;gico; nos outros vinte    dentes, ap&oacute;s este protocolo, foi tamb&eacute;m utilizado digluconato    de clorexidina a 2% (FGM<sup>&reg;</sup> Ltda., Joinville - SC, Brasil), mantido    no canal radicular por mais 3min.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Os condutos foram,    ent&atilde;o, aspirados e secados com jatos de ar e novamente subdivididos em    dois subgrupos cada em fun&ccedil;&atilde;o do material obturador a ser utilizado,    a saber, o Sealer 26<sup>&reg;</sup> (Dentsply Ind. Foram constitu&iacute;dos    quatro grupos experimentais (n = 10): Grupo I - Sealer 26<sup>&reg;</sup> com    irriga&ccedil;&atilde;o final com EDTA a 17%; Grupo II - Sealer 26<sup>&reg;</sup>    com irriga&ccedil;&atilde;o final com clorexidina a 2%; Grupo III - Sealapex<sup>&reg;</sup>    com irriga&ccedil;&atilde;o final com EDTA a 17%; Grupo IV - Sealapex<sup>&reg;</sup>    com irriga&ccedil;&atilde;o final com clorexidina a 2%.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Ap&oacute;s a impermeabiliza&ccedil;&atilde;o    externa radicular total, excluindo apenas os 2 mm apicais, com duas dem&atilde;os    de impermeabilizante de telhas (Hydronorth, Londrina - PR, Brasil) e uma camada    de esmalte para unhas, os canais radiculares dos incisivos foram preenchidos,    auxiliados por uma espiral de lentulo acionada em micromotor, com os cimentos    obturadores at&eacute; que se visualizasse uma discreta extrus&atilde;o apical.    Um cone de guta percha F<sub>1</sub> (Protaper<sup>&reg;</sup> obtura&ccedil;&atilde;o)    foi ent&atilde;o posicionado, em movimentos de vaiv&eacute;m, at&eacute; que    tamb&eacute;m ultrapassasse 2 mm apicais do &aacute;pice radicular.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">O excesso coron&aacute;rio    foi ent&atilde;o cortado e o acesso, preenchido com IRM<sup>&reg;</sup> (Dentsply    Ind. Com. Ltda, Petr&oacute;polis - RJ, Brasil) e identicamente impermeabilizado.    Todos os esp&eacute;cimes foram imersos sob v&aacute;cuo em Rhodamine B<sup>&reg;</sup>    por 72h. Ap&oacute;s, foram lavados em &aacute;gua corrente por uma hora, sendo    removidas as impermeabiliza&ccedil;&otilde;es e clivados no sentido longitudinal    na dire&ccedil;&atilde;o vestibulolingual.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">A magnitude da    infiltra&ccedil;&atilde;o apical foi obtida a partir de medidas efetuadas com    o programa Image Tool<sup>&reg;</sup> (University of Texas Health Science Center,    San Antonio, Texas, EUA) de fotografias digitalizadas dos dentes, posicionados    ao lado de um paqu&iacute;metro. Os dados foram submetidos &agrave; an&aacute;lise    estat&iacute;stica n&atilde;o param&eacute;trica de Kruskal Wallis e ao confronto    de Dunn ao n&iacute;vel de 5% de signific&acirc;ncia.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Resultados</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Os dados obtidos,    ap&oacute;s serem submetidos ao teste de Kruskal Wallis, demonstraram semelhan&ccedil;as    entre si (<i>p</i> &gt; 0,05), com exce&ccedil;&atilde;o do grupo do Sealapex<sup>&reg;</sup>;    em que o uso do digluconato de clorexidina a 2% proporcionou melhor vedamento    apical que o grupo em que apenas foi empregado o EDTA a 17% (<i>p</i> &lt; 0,05).</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">A <a href="#t1">Tabela    1</a> ilustra a mediana e os postos m&eacute;dios obtidos pelos grupos experimentais,    e a <a href="/img/revistas/rfo/v15n2/11t02.jpg">Tabela 2</a> demonstra o confronto entre os grupos    experimentais, tendo como Z cr&iacute;tico igual a 2,635, em n&iacute;vel de    signific&acirc;ncia a 5%. <body><![CDATA[<p><a name="f1"></a></p>     <p>&nbsp;</p>     <p align="center"><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b><img src="/img/revistas/rfo/v15n2/11f01.jpg"></b></font></p>     <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Discuss&atilde;o</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">A sobreobtura&ccedil;&atilde;o    &eacute; uma iatrogenia de question&aacute;vel resolu&ccedil;&atilde;o. Uma    vez constatada, &eacute; vi&aacute;vel a sua remo&ccedil;&atilde;o, pois a persist&ecirc;ncia    do cone de guta percha al&eacute;m do &aacute;pice radicular poder&aacute; favorecer    a forma&ccedil;&atilde;o de biofilme apical, que interferiria no processo de    reparo apical<sup>7</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Entretanto, nem    sempre se consegue a remo&ccedil;&atilde;o do cone de guta percha, seja imediatamente,    seja em per&iacute;odos tardios &agrave; conclus&atilde;o da obtura&ccedil;&atilde;o    endod&ocirc;ntica. Apesar de esse material ser biocompat&iacute;vel e relativamente    bem tolerado pelos tecidos periapicais<sup>6</sup>; quando fragmentado, principalmente    no ato mec&acirc;nico da remo&ccedil;&atilde;o, ocasiona uma resposta tecidual    intensa e localizada, caracterizada pela presen&ccedil;a de macr&oacute;fagos    e c&eacute;lulas gigantes<sup>8</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Nas situa&ccedil;&otilde;es    em que o risco de ultrapassagem da guta percha al&eacute;m do &aacute;pice radicular    existe, tais como em &aacute;pices incompletos, arrombamento e/ou dilata&ccedil;&atilde;o    da constric&ccedil;&atilde;o foraminal, todo cuidado deve ser tomado. Al&eacute;m    das atitudes preventivas, condutas que minimizem a possibilidade de fracasso,    caso ocorra o extravasamento, devem ser adotadas.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Ao haver a sobreobtura&ccedil;&atilde;o,    o caso deve ser analisado criteriosamente, avaliando-se os riscos da tentativa    de remo&ccedil;&atilde;o ou a prefer&ecirc;ncia por mant&ecirc;-la intacta.    Portanto, quanto menos exposta ficar a linha de cimento obturador existente    entre a abertura foraminal e o cone de guta percha extru&iacute;do, menor ser&aacute;    a possibilidade de microinfiltra&ccedil;&atilde;o de micro-organismos e trocas    de subst&acirc;ncias entre o canal radicular e os tecidos periapicais. Assim,    toda atitude que favore&ccedil;a um melhor selamento apical dever&aacute; ser    tentada.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">No presente estudo,    o uso do gluconato de clorexidina a 2% ap&oacute;s o EDTA a 17%, associado ao    Selapex<sup>&reg;</sup>; apresentou o melhor selamento apical (<i>p</i> &lt;    0,05). Contrariamente, para o Sealer 26<sup>&reg;</sup> n&atilde;o foi verificado    sinergismo entre m&eacute;todo de irriga&ccedil;&atilde;o final e cimento obturador    (<i>p</i> &gt; 0,05). A composi&ccedil;&atilde;o qu&iacute;mica do cimento e    suas caracter&iacute;sticas f&iacute;sicas, tais como escoamento, viscosidade    e tamanho das part&iacute;culas, exercem influ&ecirc;ncia na capacidade de penetra&ccedil;&atilde;o    intradentin&aacute;ria<sup>9</sup>.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">O comportamento    do Sealer 26<sup>&reg;</sup>; proporcionando bom vedamento apical, independentemente    ou n&atilde;o do uso posterior do gluconato de clorexidina a 2%, vem se somar    aos resultados obtidos por Maeda et al.<sup>10</sup> (2007). Tal caracter&iacute;stica    pode ser atribu&iacute;da &agrave; sua composi&ccedil;&atilde;o qu&iacute;mica    de origem resinosa e de baixa solubilidade<sup>11,12</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">A solubilidade    do material obturador tem papel importante nos resultados desta pesquisa, pois    a linha de cimento na abertura foraminal tende a ser sensivelmente maior que    nas condi&ccedil;&otilde;es normais.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Ao se empregar    o gluconato de clorexidina a 2% (tens&atilde;o superficial de 55,5 dinas/cm)<sup>13</sup>;    possivelmente houve uma maior penetra&ccedil;&atilde;o intradentin&aacute;ria    do cimento Sealapex<sup>&reg;</sup>. Isso torna sugestivo que, ao utilizar este    cimento, o emprego pr&eacute;vio de subst&acirc;ncias tensoativas reduziria    a infiltra&ccedil;&atilde;o marginal apical. Como par&acirc;metro de compara&ccedil;&atilde;o,    a &aacute;gua possui tens&atilde;o superficial de 72,73 dinas/cm<sup>14</sup>    e o hipoclorito de s&oacute;dio a 1%, tens&atilde;o de 60,7 dinas/cm<sup>15</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Essa condi&ccedil;&atilde;o    &eacute; observada quando se utilizam previamente &agrave; obtura&ccedil;&atilde;o    dos canais radiculares com o cimento Sealapex<sup>&reg;</sup> subst&acirc;ncias    de baixa tens&atilde;o superficial<sup>16</sup>; opostamente a quando somente    &eacute; empregado o EDTA a 17%<sup>17</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Assim, tomadas    as devidas precau&ccedil;&otilde;es para se evitar uma sobreobtura&ccedil;&atilde;o,    com base nos resultados obtidos no presente estudo, torna-se evidente que, vislumbrando    a poss&iacute;vel ocorr&ecirc;ncia da iatrogenia, j&aacute; de antem&atilde;o    todas as condutas dever&atilde;o ser seguidas a fim de minimizar a agress&atilde;o.    Nesse prop&oacute;sito, &eacute; de boa conduta empregar um cimento de boa qualidade    biol&oacute;gica associado ao uso pr&eacute;vio do EDTA a 17%, e, na sequ&ecirc;ncia,    o gluconato de clorexidina a 2%, com a finalidade de reduzir a infiltra&ccedil;&atilde;o    apical e exercer suas caracter&iacute;sticas antimicrobianas.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Conclus&otilde;es</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Diante do averiguado    no presente estudo, pode-se concluir que:</font></p> <ul>       <li><font face="Verdana, Arial, Helvetica, sans-serif" size="2">o cimento Sealer      26<sup>&reg;</sup> n&atilde;o sofre sinergismo com o emprego pr&eacute;vio      do gluconato de clorexidina a 2%;</font></li>       <li><font face="Verdana, Arial, Helvetica, sans-serif" size="2"> o cimento Sealapex<sup>&reg;</sup>      apresenta seu comportamento f&iacute;sico melhorado com o uso pr&eacute;vio      do gluconato de clorexidina a 2%, por 3min ap&oacute;s o emprego do EDTA a      17%.</font></li>     ]]></body>
