REVISÃO DE LITERATURA: Apesar de a mucosa bucal ser considerada o principal reservatório de Candida spp, este micro-organismo pode estar coagregado a bactérias do biofilme dental, sendo considerado um fator importante para o processo de colonização de bolsas periodontais. Além disso, possui vários fatores de virulência relevantes na patogênese da doença periodontal, tais como a capacidade de aderir ao epitélio e invadir o tecido conjuntivo gengival. Também pode inibir a função de neutrófilos polimorfonucleares, bem como produzir enzimas como colagenases e proteinases, que são capazes de degradar imunoglobulinas. CONSIDERAÇÕES FINAIS: Os fatores de virulência de Candida spp. associada à suscetibilidade do hospedeiro poderiam desempenhar um papel importante nas alterações inflamatórias associadas com as doenças periodontais destrutivas.]]></p></abstract>
<abstract abstract-type="short" xml:lang="en"><p><![CDATA[OBJECTIVE: The aim of this study is to present a brief literature review about the colonization of Candida spp. in periodontal pockets, their virulence factors and possible influence on the periodontal diseases. LITERATURE REVIEW: Although the buccal mucosa is considered the main reservoir of Candida spp., these microorganisms can be co-aggregate to bacteria in dental biofilm, being considered as an important factor for colonization process of periodontal pockets. In addiction, these yeasts possess some relevant virulence factors in the pathogenesis of periodontal disease, such as the capacity to adhere in the epithelium and to invade gingival connective tissues. Candida spp. Virulence factors.</font></p> <hr size="1" noshade>     <p>&nbsp;</p>     <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Introdu&ccedil;&atilde;o</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><i>O g&ecirc;nero    Candida</i> compreende um grupo de leveduras encontradas em diversos ecossistemas,    sendo a mucosa bucal considerada um dos principais reservat&oacute;rios de suas    diferentes esp&eacute;cies<sup>1</sup>. Aproximadamente metade da popula&ccedil;&atilde;o    adulta saud&aacute;vel possui <i>Candida</i> spp. na cavidade bucal. Entretanto,    micro-organismos deste g&ecirc;nero podem causar infec&ccedil;&atilde;o, comportando-se    como pat&oacute;genos oportunistas, geralmente em pacientes com imunossupress&atilde;o<sup>2,3</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Dentre as esp&eacute;cies    de <i>Candida,</i> a mais frequentemente associada a les&otilde;es bucais &eacute;    a <i>C. albicans</i>, por&eacute;m outras tamb&eacute;m t&ecirc;m sido isoladas    da saliva de indiv&iacute;duos portadores ou n&atilde;o de candidoses, incluindo    <i>C. glabrata, C. tropicalis, C. parapsilosis, C. krusei</i> e <i>C. dubiniensis</i><sup>4</sup>.    A habilidade de se transformar de levedura em formas filamentares &eacute; o    maior determinante de virul&ecirc;ncia dessa classe de micro-organismos<sup>5-7</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Esses micro-organismos    podem estar coagregados a bact&eacute;rias no biofilme dental, colonizando diferentes    s&iacute;tios, incluindo a l&iacute;ngua, o palato, as tonsilas, a mucosa labial    e a jugal<sup>8</sup>; as les&otilde;es de c&aacute;rie<sup>9</sup>; os canais    radiculares<sup>10</sup> e tamb&eacute;m bolsas periodontais, fazendo parte    do biofilme subgengival<sup>11,12</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Assim, a presen&ccedil;a    de <i>Candida</i> spp. pode ser considerada um fator importante para o processo    de coloniza&ccedil;&atilde;o de bolsas periodontais<sup>12</sup>. Possui v&aacute;rios    fatores de virul&ecirc;ncia relevantes na patog&ecirc;nese da doen&ccedil;a    periodontal, tais como a capacidade de aderir ao epit&eacute;lio e invadir o    tecido conjuntivo gengival<sup>13</sup>. Tamb&eacute;m pode inibir a fun&ccedil;&atilde;o    de neutr&oacute;filos polimorfonucleares<sup>14</sup>; bem como produzir enzimas    como colagenases e proteinases, que s&atilde;o capazes de degradar imunoglobulinas<sup>8</sup>.    Os fatores de virul&ecirc;ncia de <i>Candida</i> spp. associados &agrave; suscetibilidade    do hospedeiro poderiam desempenhar um papel importante nas altera&ccedil;&otilde;es    inflamat&oacute;rias associadas com as doen&ccedil;as periodontais destrutivas<sup>15</sup>.    Al&eacute;m disso, outros micro-organismos, como entamoebas e tricomonas<sup>16</sup>;    bacilos ent&eacute;ricos e pseudomonas<sup>17</sup>; v&iacute;rus como o epstein    barr - EBV<sup>18</sup>; citomegalov&iacute;rus humano HCMV<sup>19</sup> e o    v&iacute;rus do herpes simples tipo 1 - HSV<sup>20</sup>; est&atilde;o sendo    pesquisados atualmente com o objetivo de esclarecer seu papel na etiopatogenia    da doen&ccedil;a periodontal.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Com base nessas    evid&ecirc;ncias cient&iacute;ficas, a proposta do presente trabalho &eacute;    atualizar o cirurgi&atilde;o-dentista por meio de uma breve revis&atilde;o de    literatura a respeito da coloniza&ccedil;&atilde;o de <i>Candida</i> spp. em    bolsas periodontais, seus principais fatores de virul&ecirc;ncia e poss&iacute;vel    influ&ecirc;ncia sobre as doen&ccedil;as periodontais.</font></p>     <p>&nbsp;</p>     ]]></body>
<body><![CDATA[<p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Revis&atilde;o    de literatura</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>Fatores de risco    para infec&ccedil;&atilde;o por <i>Candida</i> spp.</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Essas leveduras    s&atilde;o encontradas comumente nas popula&ccedil;&otilde;es humanas e residem    em equil&iacute;brio com a microbiota comensal de hospedeiros<sup>21</sup>.    O estado fisiol&oacute;gico do hospedeiro tem sido reconhecido como a condi&ccedil;&atilde;o    prim&aacute;ria para a ocorr&ecirc;ncia de candidoses, mais do que a virul&ecirc;ncia    intr&iacute;nseca do micro-organismo oportunista<sup>22</sup>. A incid&ecirc;ncia    de candid&iacute;ase aumentou significativamente nas &uacute;ltimas duas d&eacute;cadas    e infec&ccedil;&otilde;es por corrente sangu&iacute;nea junto com <i>Candida</i>    spp. est&atilde;o se tornando a causa prim&aacute;ria de morbidade/mortalidade    em pacientes imunocomprometidos<sup>23</sup>. Hospedeiros imunossuprimidos e    a alta virul&ecirc;ncia de <i>C. albicans</i> s&atilde;o uma combina&ccedil;&atilde;o    ideal que favorece o seu surgimento como importante agente de doen&ccedil;as    em humanos<sup>24</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Mudan&ccedil;as    fisiol&oacute;gicas no organismo, como gravidez e inf&acirc;ncia ou idade avan&ccedil;ada,    fatores nutricionais, tratamentos com antibi&oacute;ticos de amplo espectro,    drogas imunossupressoras e corticosteroides s&atilde;o fatores que predisp&otilde;em    a infec&ccedil;&otilde;es por <i>Candida</i> spp.<sup>25,26</sup>. Altera&ccedil;&otilde;es    anatomofisiol&oacute;gicas bucais, como macroglossia, estagna&ccedil;&atilde;o    salivar, doen&ccedil;as respirat&oacute;rias e comprometimento simult&acirc;neo    da resposta imunol&oacute;gica inata e adquirida, al&eacute;m de fatores locais,    como xerostomia e aparelhos prot&eacute;ticos, s&atilde;o condi&ccedil;&otilde;es    que predisp&otilde;em ao desenvolvimento de infec&ccedil;&otilde;es e aumentam    a suscetibilidade a processos infecciosos, inclusive &agrave;queles causados    por leveduras do g&ecirc;nero <i>Candida</i><sup>27,28</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Al&eacute;m desses    fatores, a hipofun&ccedil;&atilde;o das gl&acirc;ndulas salivares, a diminui&ccedil;&atilde;o    do fluxo ou do pH salivar e diabetes melito, pacientes com c&acirc;ncer em est&aacute;gio    avan&ccedil;ado e HIV positivos tamb&eacute;m constituem fatores de risco para    a aquisi&ccedil;&atilde;o de candidoses. A deficiente resposta imune de pacientes    HIV positivos, associada &agrave; progress&atilde;o da infec&ccedil;&atilde;o    viral, &eacute; considerada um fator preditivo para o desenvolvimento de candidoses<sup>29</sup>.    J&aacute; estudos em pacientes diab&eacute;ticos insulino-dependentes demonstraram    uma maior preval&ecirc;ncia de <i>Candida</i> na cavidade bucal, quando comparados    aos pacientes n&atilde;o diab&eacute;ticos. Sardi et al.<sup>30</sup> (2008)    mostraram que os pacientes diab&eacute;ticos com periodontite cr&ocirc;nica    tiveram maior preval&ecirc;ncia de <i>Candida</i> quando comparados aos n&atilde;o    diab&eacute;ticos.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>Biofilme formado    por <i>Candida</i> spp.</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">O biofilme pode    ser definido como um conjunto de micro-organismos firmemente aderidos a uma    superf&iacute;cie, envoltos por uma matriz extracelular composta por polissacar&iacute;deos,    prote&iacute;nas e &aacute;cidos nucleicos produzidos por eles. Consiste numa    forma s&eacute;ssil e adaptativa no ciclo biol&oacute;gico dos micro-organismos,    com caracter&iacute;sticas genot&iacute;picas e fenot&iacute;picas que o distinguem    das formas microbianas livres e circulantes, denominadas de formas planct&ocirc;nicas<sup>31</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">O biofilme de <i>Candida</i>    possui importante repercuss&atilde;o cl&iacute;nica por causa do aumento da    resist&ecirc;ncia a drogas, pela sua habilidade de resistir ao sistema imune    do hospedeiro e aderir a dispositivos m&eacute;dicos de implantes e odontol&oacute;gicos.    Dispositivos implantados, como cateteres, v&aacute;lvulas prot&eacute;ticas    do cora&ccedil;&atilde;o, assim como dispositivos relacionados e pr&oacute;teses    dentais, permitem a este pat&oacute;geno aderir &agrave; superf&iacute;cie,    formando biofilme<sup>32</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">A forma&ccedil;&atilde;o    do biofilme de <i>Candida</i> ocorre em diferentes fases, que incluem a ader&ecirc;ncia    inicial e a coloniza&ccedil;&atilde;o, seguida da divis&atilde;o celular, prolifera&ccedil;&atilde;o    e matura&ccedil;&atilde;o do biofilme<sup>33</sup>. Esta inclui formas leveduriformes    e filamentares, adequadas para a coloniza&ccedil;&atilde;o de superf&iacute;cies    inertes, sendo a forma leveduriforme a produtora de uma capa basal adesiva e    a forma filamentar a que prov&ecirc; o suporte para o desenvolvimento de uma    estrutura multiextrato<sup>34</sup>. Durante a fase de ades&atilde;o, forma-se    uma camada de c&eacute;lulas leveduriformes flutuantes a uma superf&iacute;cie    e, sobre estas, uma camada de formas filamentares rodeadas por uma matriz de    polissacar&iacute;deos<sup>33</sup>. O material extracelular produzido pela    esp&eacute;cie <i>C. albicans</i> cont&eacute;m carboidratos, prote&iacute;nas    e outros componentes ainda desconhecidos, sendo esta composi&ccedil;&atilde;o    diferente das c&eacute;lulas n&atilde;o aderidas (planct&ocirc;nicas)<sup>34</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">O biofilme maduro    de <i>C. allbicans</i> mostra uma complexa arquitetura tridimensional e extensiva    heterogeneidade espacial, que representa uma otimiza&ccedil;&atilde;o do espa&ccedil;o    para facilitar o afluxo de nutrientes entre os micro-organismos para elimina&ccedil;&atilde;o    dos metab&oacute;litos, al&eacute;m do estabelecimento de micronichos<sup>35,36</sup>.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>Detec&ccedil;&atilde;o    de <i>Candida</i> spp. em bolsas periodontais</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">As doen&ccedil;as    periodontais s&atilde;o caracterizadas por acometerem os tecidos de prote&ccedil;&atilde;o    e suporte dos dentes, e micro-organismos presentes no biofilme subgengival induzem    a uma variedade de respostas imunesdefensivas no hospedeiro, na tentativa de    conter a infec&ccedil;&atilde;o<sup>12</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Muitas pesquisas    t&ecirc;m sido realizadas visando &agrave; detec&ccedil;&atilde;o de <i>Candida</i>    spp. em bolsas periodontais, com &ecirc;nfase na esp&eacute;cie <i>C. albicans</i>,    de modo a verificar a capacidade dessas esp&eacute;cies de sobreviverem nesses    microambientes e com o objetivo de esclarecer seu papel na patog&ecirc;nese    da doen&ccedil;a periodontal. Apesar de a doen&ccedil;a periodontal ser causada    por diversas esp&eacute;cies de bact&eacute;rias anaer&oacute;bias Gram-negativas,    <i>C. albicans</i> foi isolada em 20% das bolsas periodontais dos pacientes    com periodontite. Esse estudo avaliou a poss&iacute;vel correla&ccedil;&atilde;o    entre o isolamento subgengival de <i>C. albicans</i> e a ocorr&ecirc;ncia de    determinadas esp&eacute;cies periodontopatog&ecirc;nicas, observando que <i>P.    gingivalis</i> foi menos frequentemente isolada em amostras positivas para <i>C.    albicans</i>; ao contr&aacute;rio, a propor&ccedil;&atilde;o m&eacute;dia de    <i>A. actinomycetemcomitans</i> em rela&ccedil;&atilde;o &agrave; microbiota    total era maior nos indiv&iacute;duos positivos do que naqueles em que tais    leveduras n&atilde;o foram encontradas nas bolsas periodontais<sup>11</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Diversos estudos    visando &agrave; detec&ccedil;&atilde;o de <i>Candida</i> spp. em bolsas periodontais    de pacientes que possuem fatores de risco para candidoses foram realizados.    Portela et al.<sup>37</sup>(2004) e Feller e Lemmer<sup>38</sup>(2008) relataram    aumento na preval&ecirc;ncia das esp&eacute;cies de <i>Candida</i> na cavidade    bucal de pacientes soropositivos, especificamente em biofilme subgengival. Nos    pacientes diab&eacute;ticos, Sardi et al.<sup>30</sup> (2008) encontraram quatro    esp&eacute;cies de <i>Candida</i>, sendo <i>C. dubliniensis</i> e <i>C. albicans</i>    as mais prevalentes nos s&iacute;tios periodontais, al&eacute;m de <i>C. glabrata</i>    e <i>C. krusei</i>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Assim, <i>C. albicans</i>    tem sido relatada em diferentes manifesta&ccedil;&otilde;es da doen&ccedil;a    periodontal, incluindo a periodontite agressiva<sup>8</sup>; a periodontite    cr&ocirc;nica<sup>30</sup>; s&iacute;tios refrat&aacute;rios &agrave; terapia    periodontal e bolsas periodontais de fumantes<sup>39</sup> e diab&eacute;ticos<sup>30</sup>.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Discuss&atilde;o</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><i>C. albicans</i>    &eacute; considerada a esp&eacute;cie mais virulenta e adaptada &agrave; cavidade    bucal, sobrevivendo e proliferando em v&aacute;rios nichos ecol&oacute;gicos    fornecidos pelo hospedeiro humano<sup>40</sup>. Seus diversos fatores de virul&ecirc;ncia,    discutidos a seguir, apontam para evid&ecirc;ncias cient&iacute;ficas de <i>Candida</i>    spp. na patog&ecirc;nese das doen&ccedil;as periodontais.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>Capacidade de    participar do biofilme dental</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">O biofilme oferece    prote&ccedil;&atilde;o contra os mecanismos de remo&ccedil;&atilde;o pela saliva    e dificulta a a&ccedil;&atilde;o de antimicrobianos<sup>41</sup>; al&eacute;m    de servir como reservat&oacute;rio para libera&ccedil;&atilde;o de organismos    infectantes na cavidade bucal. Desse modo, o biofilme contendo leveduras poderia    estar implicado n&atilde;o apenas nas candidoses em mucosas, mas tamb&eacute;m    no desenvolvimento de c&aacute;ries<sup>42</sup> e na patog&ecirc;nese das doen&ccedil;as    periodontais<sup>12</sup>.</font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">A coagrega&ccedil;&atilde;o    a outros micro-organismos bucais tamb&eacute;m constitui um fator importante    na progress&atilde;o de doen&ccedil;as bucais. A associa&ccedil;&atilde;o ocorre    por meio da ades&atilde;o de receptores de membrana, incluindo a manoprote&iacute;na    da <i>C. albicans</i><sup>43</sup>. As manoprote&iacute;nas est&atilde;o envolvidas    no reconhecimento c&eacute;lula-c&eacute;lula e respostas aos fatores de estresse,    merecendo destaque como potencial fator agressivo e principal alvo da resposta    imune do hospedeiro, favorecendo a ades&atilde;o do fungo &agrave; superf&iacute;cie    da c&eacute;lula hospedeira<sup>44</sup>. Por ser uma mol&eacute;cula que cont&eacute;m    sacar&iacute;deos e prote&iacute;na, serve como adesina durante a coloniza&ccedil;&atilde;o    e infec&ccedil;&atilde;o<sup>45</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>Capacidade de    ades&atilde;o a c&eacute;lulas do hospedeiro</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">A ades&atilde;o    de micro-organismos &agrave;s superf&iacute;cies mucosas do hospedeiro &eacute;    pr&eacute;-requisito para a coloniza&ccedil;&atilde;o e subsequente estabelecimento    da infec&ccedil;&atilde;o nos tecidos mucosos<sup>46</sup>; evitando o deslocamento    do micro-organismo pela a&ccedil;&atilde;o mec&acirc;nica da saliva<sup>47,48</sup>.    Assim, a ades&atilde;o &eacute; importante para a persist&ecirc;ncia e dissemina&ccedil;&atilde;o    infecciosa, tanto em tecidos bucais como nos sist&ecirc;micos<sup>49</sup>.    Staab et al.<sup>50</sup> (1999) relataram que linhagens de <i>C. albicans</i>    que apresentam ades&atilde;o diminu&iacute;da &agrave;s c&eacute;lulas epiteliais    humanas possuem capacidade reduzida de causar candidose sist&ecirc;mica em camundongos.    Al&eacute;m disso, alguns estudos<sup>51,52</sup> observaram melhor ades&atilde;o    das c&eacute;lulas de <i>C. albicans</i> em superf&iacute;cies acr&iacute;licas    quando essas leveduras cresceram em meios suplementados com maiores concentra&ccedil;&otilde;es    de galactose, sucrose, glucose ou maltose. Essa &eacute; uma observa&ccedil;&atilde;o    relevante por considerar que uma dieta rica em carboidratos pode predispor o    indiv&iacute;duo a infec&ccedil;&atilde;o bucal por <i>C. albicans</i>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Prote&iacute;nas    hidrof&oacute;bicas formadas pela <i>C. albicans</i> na matriz polissacar&iacute;dica    tamb&eacute;m aumentam a for&ccedil;a dos receptores de ades&atilde;o, contribuindo    para a patogenicidade dessa esp&eacute;cie de levedura<sup>43</sup>. Quanto    mais hidrof&oacute;bicas, mais aderentes &agrave;s c&eacute;lulas do hospedeiro    e aos componentes salivares, como mucina e matriz de prote&iacute;nas extracelulares<sup>53</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>Invas&atilde;o    do tecido conjuntivo gengival</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Quanto &agrave;    intera&ccedil;&atilde;o de <i>Candida</i> spp. com as c&eacute;lulas epiteliais,    acredita-se que a produ&ccedil;&atilde;o de hifas aumente o poder de ader&ecirc;ncia    e de invas&atilde;o tecidual,</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">permitindo &agrave;s    leveduras o acesso a estruturas e &oacute;rg&atilde;os na profundidade do corpo<sup>40</sup>.    A forma de blastosporos tamb&eacute;m tem sido encontrada na superf&iacute;cie    celular, bem como entre as c&eacute;lulas do hospedeiro<sup>54</sup>. J&auml;rvensivu    et al.<sup>12</sup> (2004) demonstraram a capacidade de invas&atilde;o das hifas    de <i>C. albicans</i> aos tecidos periodontais em tecidos acometidos por periodontite    cr&ocirc;nica e no biofilme subgengival <i>in vivo</i> por meio da t&eacute;cnica    com anticorpos monoclonais. Al&eacute;m disso, estudos sugerem que a prote&iacute;na    SAP (aspartil proteinase secretada), produzida pela <i>C. albicans</i>, &eacute;    capaz de digerir a superf&iacute;cie da c&eacute;lula epitelial do hospedeiro,    provocando lise e consequente invas&atilde;o tecidual<sup>54</sup>. As SAPs    podem se adaptar e se envolver diretamente em v&aacute;rias fun&ccedil;&otilde;es    de virul&ecirc;ncia. Por exemplo, SAPs podem contribuir para a ades&atilde;o    e invas&atilde;o do tecido hospedeiro, degradando as estruturas da superf&iacute;cie    da c&eacute;lula e subst&acirc;ncias intercelulares ou destruindo c&eacute;lulas    e mol&eacute;culas do sistema imune como uma maneira de resistir ao ataque da    resposta imune do hospedeiro<sup>55</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>Indu&ccedil;&atilde;o    de rea&ccedil;&otilde;es inflamat&oacute;rias</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Fazendo parte da    microbiota subgengival, <i>Candida</i> spp. geralmente provoca inflama&ccedil;&atilde;o    tecidual. J&auml;rvensivu et al.<sup>12</sup> (2004) detectaram <i>Candida</i>    spp. em diversas camadas do biofilme subgengival, incluindo a presen&ccedil;a    de infiltrado inflamat&oacute;rio. Nas camadas mais externas, pareceu agir como    uma barreira de prote&ccedil;&atilde;o dos micro-organismos das camadas mais    internas do biofilme. Desse modo, os autores sugerem que <i>Candida</i> spp.    pode conferir prote&ccedil;&atilde;o a esses micro-organismos diante dos mecanismos    imunes do hospedeiro, auxiliando na resist&ecirc;ncia da microbiota subgengival,    al&eacute;m de contribuir para a persist&ecirc;ncia da resposta inflamat&oacute;ria    nos tecidos adjacentes. <i>C. albicans</i> tamb&eacute;m tem papel na evas&atilde;o    do sistema imune, podendo provocar uma infec&ccedil;&atilde;o periodontal com    destrui&ccedil;&atilde;o tecidual<sup>12</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>Resposta a altera&ccedil;&otilde;es    ambientais e <i>quorum sensing</i></b></font></p>     ]]></body>
<body><![CDATA[<p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Esta capacidade    de adapta&ccedil;&atilde;o pode ser explicada pelo mecanismo de <i>quorum sensing</i>,    que pode ser definido como um sistema complexo de comunica&ccedil;&atilde;o    intercelular por meio de sinaliza&ccedil;&atilde;o molecular. A sinaliza&ccedil;&atilde;o    se d&aacute; em raz&atilde;o de flutua&ccedil;&otilde;es ambientais, tais como    diferen&ccedil;a de pH, temperatura, disponibilidade de nutrientes, entre outros.    Esse fen&ocirc;meno ocorre porque esp&eacute;cies de <i>Candida</i> possuem    sistemas que permitem sua adapta&ccedil;&atilde;o em resposta a altera&ccedil;&otilde;es    ambientais<sup>56</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">A bolsa periodontal    e o fluido crevicular s&atilde;o ambientes favor&aacute;veis para germina&ccedil;&atilde;o    e crescimento de hifas de <i>Candida</i> spp. Como j&aacute; explicado, na forma    de hifa <i>Candida</i> spp. tem maior capacidade de penetrar nos tecidos e aderir    &agrave; superf&iacute;cie epitelial do hospedeiro, em compara&ccedil;&atilde;o    com a forma de levedura<sup>12</sup>.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">Pizzo et al.<sup>3</sup>    (2002) sugeriram que a heterogeneidade de cepas de <i>Candida</i> na bolsa subgengival    n&atilde;o &eacute; apenas resultado do seu espalhamento na saliva ou no biofilme,    mas tamb&eacute;m da sua capacidade de adapta&ccedil;&atilde;o &agrave; bolsa,    desenvolvendo diferentes propriedades de virul&ecirc;ncia para sua sobreviv&ecirc;ncia    em ambientes hostis.</font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><b>Secre&ccedil;&atilde;o    de enzimas que degradam tecidos</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2"><i>C. albicans</i>    pode expressar na superf&iacute;cie celular a secre&ccedil;&atilde;o de proteinases    capazes de degradar a matriz extracelular e componentes da membrana basal do    epit&eacute;lio, como o col&aacute;geno e a fibronectina. Dentre as importantes    enzimas de <i>Candida</i> spp. que degradam tecidos est&atilde;o as fosfolipases    e aspartil proteinases secretadas (SAPs). Sete tipos de genes de fosfolipases    (PLA, PLB1, PLB2, PLC1, PLC2, PLC3 e PLD1) e 10 de SAP (SAP1 a SAP10) t&ecirc;m    sido identificados em <i>C. albicans</i><sup>57</sup>. Apesar de o seu papel    na patog&ecirc;nese da doen&ccedil;a periodontal n&atilde;o ter sido esclarecido,    sabe-se que as fosfolipases podem estar envolvidas na invas&atilde;o tecidual    por degradar fosfolip&iacute;dios, causando lise na membrana da c&eacute;lula    do hospedeiro<sup>57</sup>.</font></p>     <p>&nbsp;</p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="3"><b>Considera&ccedil;&otilde;es    finais</b></font></p>     <p><font face="Verdana, Arial, Helvetica, sans-serif" size="2">O aumento na preval&ecirc;ncia    de esp&eacute;cies de <i>Candida</i> na bolsa periodontal, principalmente de    <i>C. albicans</i>, sugere sua coparticipa&ccedil;&atilde;o na patog&ecirc;nese    da doen&ccedil;a periodontal.
