Although the high success rate of endodontic treatments of teeth with apical periodontitis, failures can occur. These clinical cases can be treated by conventional endodontic retreatment, or extraction and in some clinical situations, through endodontic microsurgery. With a success rate above 90%, the endodontic microsurgery is a good therapy to be considered in order to maintain these teeth in function.
