Perforations located in the apical perimeter during endodontic instrumentation are related to negligence or ignorance of internal anatomical variations of the dental root canal. We describe a case report of perforation in the middle third of the root, where the initial treatment was based on the decontamination and sealing the cavity of perforation with aggregate mineral trioxide (MTA), and finished with sealing the root perfora tion with gutta-percha and root canal sealer. The clinical control of 12 months presented a satisfactory rehabilitation associated with periodontal health of area. It can be concluded that sealing the perforation had an outcome highlighted by excellent repair.
