Introduction: Odontomas are the most commom and often diagnosed odontogenic tumors in the second decade of life. Its slow growth, benign and asymptomatic behavior favors long staying intraosseous until show any clinical sign or be detected in routine radiographs, may reach large dimensions. Case report: The objective of this report is to demonstrate a case of large complex odontoma in mandible, uncovered by the swelling in the mandibular angle, as well as conservative surgical treatment, associated with the installation of Erich bar, a maxillo-mandibular block for 30 days and monitoring of pathological fracture with spontaneous reduction. Proservation of 6 years was achieved with no sign of recurrence. Final considerations: The importance of the dentist's knowledge about the epidemiological profile of his entity is emphasized. It is evident that the radiographic investigation in unerupted teeth, delayed exfoliation of deciduous teeth or permanent teeth ectopic position can make early diagnosis easier. The most conservative treatment is recommended since recurrence is rare.
