ABSTRACT Introduction: the oral surgery with removal of bone and deep sections on the soft tissues obviously cause significant changes in these structures, with highlights to the vascular ones, including vessels that irrigate directly the dental pulp. Depending on the extension of the surgical site, it is possible to stop the irrigation of the pulp tissue. Such changes are confirmed in literature, especially in Le Fort I surgeries, intermaxillary sutures and osteotomies. Case report: it was described and discussed a case from the ambulatory of the course of bucomaxillofacial surgery residency, in which, after removal of the mento's bone tissue to graft in the maxilla, two teeth showed color change. It was, subsequently, evidenced the aseptic necrosis of these teeth, which received endodontic treatment and internal bleaching procedure. Final considerations: in surgical procedures where it happens a deep intervention in face's blood vessels are required rigorous planning allied to post-surgical monitoring due to, among other possibilities, pulp changes.
