The osseointegration technique revolutionizes the Dentistry and brings advantages compared to the conventional treatment preserving remaining teeth, better retention and stability to the rehabilitations, with foreseeable results and stable in time. Despite having high success rates (more than 90%), some failures could occurs and in this context, the aim of this article was to enhance the local and systemic conditions related with the patients that could affect the osseointegration process. After analysis and literature review from 1969 to 2007, it was verified that smoking habits, irradiation, diabetes, periodontal disease, osteoporosis, age and inadequate bone quality were the main factors related with the patients that could affect the osseointegration. In addition, the failure rate verified were low and it was not observed absolute contra-indication to the treatment, but some conditions were considered of high risk (smoking habits and tissue irradiated) and should be considered during the steps of planning and advise the patients previously to the treatment.
