INTRODUCTION: The C-shaped molar is an anatomical variation that deserves careful attention in endodontic treatment due to its characteristic peculiarities. OBJECTIVE AND CASE REPORT: This study aimed to review the scientific literature on this morphological alteration and to report a case of endodontic treatment of a C-shaped second molar. CONCLUSION: A good planning of all stages of the treatment is essential to reach success in endodontic treatment in C-shaped molars.
