The primary stability of implants is an important factor for osseointegration successful and justifies the application of prefabricated bars in edentulous mandible when the use of immediate loading. However in some situations this alternative treatment is not indicated, and the professional know the anatomic characteristics of the arch of the patient to have safety in using prefabricated bar. The purpose of this article was to describe a clinical report using a prefabricated bar in edentulous mandible prepared to receive an immediate loading implant-supported prosthesis type protocol with clinical follow-up of 2 years. The Neopronto system (Neodent Implante Osseointegrável, Neodent, Curitiba, PR, Brazil) was used to drive four parallel inter-foraminal implants and facilitate the installation of the immediate loading prosthesis. It can be conclude that the use of this prefabricated bar technique is a viable alternative treatment for edentulous mandible, because it allows a level of adaptability and passivity predictable, reducing clinical time. However, his indication is accurate for cases in which the shape of the arch of the patient perfectly suits technique.
